摘  要:目的:探讨癌基因K-ras点突变与非小细胞肺癌(NSCLC)原发灶及淋巴结癌转移灶经典HLA-Ⅰ类抗原表达下调的关系。方法:应用流式细胞术检测65例NSCLC原发灶及31例淋巴结转移灶经典HLA-Ⅰ类抗原的表达;应用PCR-RFLP技术检测K-ras基因第12位密码子点突变(正常肺组织及淋巴结组织作对照);根据不同的变量类型及目的,采用t检验、Spearman相关分析和χ2检验等进行统计分析。结果:淋巴结转移灶经典HLA-Ⅰ类抗原的表达率为(15.35±6.24)%,明显低于NSCLC原发灶的(39.68±12.46)%,t=2.06,P=0.034;且与NSCLC原发灶经典HLA-Ⅰ类抗原表达下调明显呈正相关,rs=0.487,P=0.008。经典HLA-Ⅰ类抗原表达阳性的NSCLC原发灶中未检测到K-ras基因突变,经典HLA-Ⅰ类抗原阴性表达者的K-ras基因突变率为54.5%(6/11),低表达者的K-ras基因突变率为20.0%(9/45),差异有统计学意义,χ2=5.38,P=0.042。结论:经典HLA-Ⅰ类抗原表达下调是NSCLC淋巴结转移的机制之一,癌基因K-ras突变与NSCLC原发灶及淋巴结转移灶经典HLA-Ⅰ类抗原低表达有关。OBJECTIVE: To probe the relationship between the mutation of K-ras oncogenes and down-regulation or loss of classical HLA class Ⅰ antigen in primary lesions of non-small cell lung cancer and metastatic lesions of lymph nodes. METHODS: Expression of classical HLA class Ⅰ antigens was examined by flow cytometry (FCM) in 65 primary lesions of NSCLC and 31 metastatic cancer lesions in lymph node. The detection of K ras mutations were performed by PCR-RFLP in 65 primary lesions and 31 metastatic lesions. The results were analysed by t test,Spearman correlation analysis and g.e test. RESULTS: Expression rate of classical HLA class Ⅰ antigens in 31 lymph node metastatic lesions (15.35±6.24) % was remarkably lower than that in primary lesions (39.68±12.46) % (t=2.06,P=0. 034),and positive correlation between down-regulation of classical HLA class Ⅰ antigens in primary lesions in lung cancer and metastatic lesions in lymph nodes was discovered (rs =0. 487,P=0. 008). The difference of incidences of K ras gene mutations between primary lesions with down-regulation of classical HLA class Ⅰ antigen (20.0%) and loss of classical HLA class antigen (54.5 % ) was significant (χ^2= 5.38, P= 0. 042). CONCLUSIONS: Loss or down-regulation of classical HLA class I antigen expression in primary lesions is one of mechanisms of cancer metastasis to lymph node in NSCLC. K- ras gene mutations is related to calssical HLA-Ⅰantigens loss or downregulation in primary lesions and metastatic lesions in NSCLC.
