大肠癌、大肠腺瘤CyclinD1、P16和PCNA蛋白表达及其意义

摘　　要：目的　探讨细胞周期调控因子蛋白表达在大肠癌发生发展中的作用及其对预后影响。方法　采用免疫组化染色对 113例大肠癌、14例大肠腺瘤、11例腺瘤癌变组织中CyclinD1、P16和PCNA进行了检测。结果　大肠癌、腺瘤和癌变CyclinD1、P16以及PCNA阳性表达率与正常大肠粘膜相比均相差显著 (P <0 .0 5 ) ,腺瘤癌变CyclinD1表达明显增高 ,大肠癌P16表达明显降低。大肠癌、腺瘤癌变的PCNA较腺瘤组显著增高 (P <0 .0 1)。CyclinD1、PCNA过度表达者 ,其 5年生存率均显著降低。结论　CyclinD1过度表达与P16基因突变失活可能加速细胞周期转化 ,促进大肠癌的发生 ;CyclinD1过度表达在大肠癌早期形成过程中可能起一定作用。CyclinD1、PCNA过度表达提示为大肠癌预后不良的危险因素。Objective To study the effect of the expression of cell cycle regulators on the development and prognosis of colorectal carcinoma. Methods The expressions of Cyclin D1, P16 and proliferation cell nuclear antigen (PCNA) were detected with immunohistochemical staining in the tissues of 14 cases of adenoma, 11 cases of adenoma with malignant degeneration and 113 cases of carcinoma in large intestine. Results The positive rate of Cyclin D1, P16 and PCNA were significantly different between colorectal carcinoma, adenoma, adenoma with malignant degeneration and normal mucosa ( P <0.05). Overexpression of Cyclin D1 was found in adenoma with malignant degeneration and underexpression of P16 in carcinoma. Expression of PCNA was increased more obviously in carcinoma and adenoma with malignant degeneration than in adenoma ( P <0.01). The 3 or 5 year survival rate was significantly lower in the patients with a tumor of strong Cyclin D1 and PCNA expression than in the patients with a tumor of negative expression of Cyclin D1 and PCNA. Conclusion The overexpession of Cyclin D1 and inactivation of the mutation of P16 gene might accelerate the progression of the cell cycle and promote the carcinogenesis of the large intestine. Overexpression of Cyclin D1 may play a role in the initiation of colorectal carcinoma. The overexpessions of Cyclin D1 and PCNA predict unfavorable prognosis of colorectal carcinoma.
