老年肺癌外科治疗及临床分析

摘　　要：目的 :探讨老年肺癌外科治疗的特点。方法 :对 1983年至 1999年我院收治的 79例老年肺癌的外科治疗进行了临床分析。结果 :根治性切除 54例占 6 8.4 % ,姑息性切除 2 2例占 2 7.8% ,单纯开胸探查 3例占 3.8%。肺叶切除 56例 ,支气管袖状加肺叶切除 9例 ,全肺切除 6例 ,局部切除 5例 ,切除率 96 .2 %。手术并发症 2 1例占 2 6 .6 % ,其中呼吸系统并发症 14例占 17.7% ,肺炎 9例 ,肺不张 5例 ,循环系统 5例占 6 .3%。心律失常为主 ,心功能不全 1例。手术后死亡 1例 (1.2 6 % ) ,5年生存率35.6 %。结论 :强调老年肺癌术前更应注重肿瘤的分期、病理类型、心肺功能和合并疾病 ,重视呼吸和循环系统的准备和处理 ,手术以肺叶切除为主 ,强调尽量施行袖式肺叶切除以替代全肺切除 。Objective:[WT5BZ]To study characteristics of surgical Treatment of the aged Lung Cancer.[WT5HZ]Methods:[WT5BZ]We analysis of surgical treatment of the aged lung cancer from 1983 to 1999 in our hospital.[WT5HZ]Results:[WT5BZ]The mode of performed consisted of:regular lobectomy 56 cases ,sleeve lobectomy 9 cases,pneumonectomy 6 cases,wedge and segmentectomy 5 cases,The overall resectability was 96.2%.operative morbidity was 21 cases (26.6%).and the 30 day mortality was 1 case(1.26%).a 5 yr survival rate of 35.6%.[WT5HZ]Conclusion:[WT5BZ]Authors emphasize the substitution of pneumonectomy by sleeve lobectomy whenever identical radicality could be achieved.
