DNA含量、P^(53)基因蛋白表达在甲状腺滤泡性肿瘤诊断中的意义

摘　　要：目的　DNA含量、P53 表达和探讨核仁形成区嗜银蛋白 (AgNOR)计数在甲状腺滤泡性肿瘤诊断中的意义。方法　应用胶银染色法行AgNOR计数、流式细胞术 (FCM)行细胞核DNA含量测定 ,间接免疫荧光标记通过FCM行P53 的定性和定量检测 ,分析AgNOR个数、DNA含量及P53 基因蛋白表达与滤泡性肿瘤的组织学类型的关系。结果　AgNOR计数在正常甲状腺上皮、腺瘤与腺瘤伴上皮增生活跃、腺癌间差异有显著性 (P <0 .0 5 ) ,腺瘤伴上皮增生活跃与腺癌间差异无显著性。DNA含量在正常甲状腺与腺瘤之间差异无显著性 ,正常甲状腺上皮、腺瘤与腺瘤伴上皮增生活跃、腺癌间差异有显著性 (P <0 .0 1) ,而腺瘤伴上皮增生活跃与腺癌之间差异无显著性。P53 基因蛋白表达在正常上皮与腺瘤间差异无显著性 ;与腺瘤伴上皮增生活跃、腺癌间差异有显著性 (P <0 .0 1)。腺瘤与腺瘤伴上皮增生活跃和腺癌间差异有显著性 (P <0 .0 5 )。而腺瘤伴上皮增生活跃与腺癌间差异无显著性。结论　在甲状腺滤泡性肿瘤的鉴别中行AgNOR计数意义不大 ,而行DNA含量测定及P53基因蛋白的定量有一定的意义。Objective To study the diagnostic significance of DNA content, P 53 protein and argyrophil nucleolar organizer region (AgNOR) count in the follicular neoplasm of the thyroid gland. Methods AgNORs were counted and DNA content and P 53 protein were investigated by flow cytometry (FCM). Relation among AgNOR count, DNA content, P 53 protein and the histologic types of the thyroid tumors was analysed. Results There was significant difference between Ⅰ (normal thyroid tissue)、Ⅱ (adenoma) and Ⅲ (adenoma with active epithelial proliferation)、 Ⅳ (follicular carcinoma) in AgNOR count (P<0.05), DNA content (P<0.01) and P 53 gene protein expression (P<0.05). But no difference between Ⅰ and Ⅱ, as well as Ⅲ and Ⅳ was found. Conclusion Adenoma with active epithelial proliferation is a borderline lesion. It is rather valuable to study DNA content and P 53 protein in the differential diagnosis of the follicular neoplasms of the thyroid than to study AgNOR count.
