摘  要:目的观察脑梗死及Ⅱ型糖尿病患者血小板膜糖蛋白及纤维蛋白原结合的改变,确定其在血栓及血栓前状态研究中的价值。方法应用流式细胞术测定85例Ⅱ型糖尿病患者和28例脑梗死患者血小板纤维蛋白原结合率及糖蛋白(GP)Ⅰb、GPⅡb、GPⅢa、GPⅡb-Ⅲa复合物及P-选择素的表达,并与30例健康成人所测结果进行比较。结果血小板膜P-选择素,纤维蛋白原结合率及GPⅡb-Ⅲa复合物的表达在脑梗死患者及糖尿病伴血管病变组明显高于正常组;GPⅠb则低于正常组;GPⅡb与Ⅲa在各组间表达均无显著差异;而糖尿病无血管病变组与正常组比较无明显改变。结论脑梗死与糖尿病伴血管病变患者血小板膜纤维蛋白原结合率及P-选择素、GPⅡb-Ⅲa复合物表达升高,而GPⅠb降低。Objective To investigate the change of platelet membrane glycoproteins and its fibrinogen bind- ing reaction in patients with cerebral infarction and type Ⅱ diabetes mellitus (DM), and assess their value in thestudy of thrombosis and pro-thrombosis state. Methods By flow cytometry, the platelet glycoprotein (GP) Ⅰ b,GP Ⅱ b, GP Ⅲ a, GP Ⅱ b- Ⅲ a complex and P-selectin expression as well as fibrinogen binding reaction were analyzedin 85 patients with type Ⅱ DM and 28 with cerebral infarction, and compared with that in 30 normal subjects.Re-sults The platelet membrane GP Ⅱ b- Ⅲ a complex, P-selectin expression and fibrinogen binding reaction were higher, while the platelet surface GP Ⅰ b was lower in the patients with cerebral infartion and type Ⅱ DM with vas-cular disease than those in normal group. The GP Ⅱ b and GP Ⅲ a were not significantly changed. There was no dif-ference of these paremeters between the type Ⅱ DM without vascular disease patients and normal group. Conclusion The platelet fibrinogen binding reaction, P-selectin and GP Ⅱ b-Ⅲ a complex expression were increased while theGP Ⅰ b decreased in patients with cerebral infartion and type Ⅱ DM with vascular disease.
