幽门螺杆菌感染对胃癌及癌前病变中PCNA表达的影响

摘　　要：目的　探讨胃粘膜幽门螺杆菌 (Hp)感染与胃癌发生的关系。方法　采用免疫组化SP法检测基本正常胃粘膜 (对照组 ,3 0例 )、肠化生 ( 3 0例 )、不典型增生 ( 2 2例 )及胃癌 ( 2 5例 )组织中细胞核增殖抗原 (PCNA )的表达 ,采用快速尿素酶反应检测Hp感染情况。结果　PCNA表达与胃粘膜病变程度呈正相关 (γ =0 .65 5 ,P <0 .0 1) ;各组间Hp感染率无显著性差异 ,在肠化生及不典型增生组中Hp感染者的PCNA表达明显高于Hp阴性者 (P <0 .0 5 )。 结论　Hp感染可能通过影响细胞的增殖状况参与胃癌发生 ,其作用主要表现在癌前病变阶段。Objective To study the relationship between Helicobacter pylori(Hp) infection and carcinogenesis of gastric carcinoma.Methods 30 cases of normal mucosa,30 intestinal metaplasia(IM),22 dysplasia(Dys) and 25 gastric adenocarcionma(GAC) were studied.Hp infection was detected by rapid urease test,the expressions of PCNA were detected by immunohistochemical techniques(SP).Results There was a positive correlation between the expressions of PCNA and the degree of gastric mucosa lesions(r=0.665, P <0.05).No significant difference was found among all groups in Hp infection rates( P >0.05).In IM and Dys groups,the expressions of PCNA in Hp(+) tissues were higher than those in Hp(-) tissues( P <0.05).Conclusion Hp infection may play a part in gastric carcinogenesis through effecting the gastric epithelial proliferation,and its action seems to express on the precancerous lesions.
