摘 要:目的探讨应激性胃溃疡中Th1/Th2细胞和Th17/Treg细胞平衡的变化及凯时的干预作用。方法将40只雄性SD大鼠随机分为5组:正常对照组(N组)、应激组(S组)、脂微球空载体10μg/kg组(L组)、凯时低剂量1μg/kg组(M组)和凯时高剂量10μg/kg组(P组)。大鼠经上述药物预处理后,制作应激性胃溃疡模型。造模完成后采集外周血分离淋巴细胞及血浆,采用荧光定量PCR方法分析Th1/Th2/Th17/Treg细胞相对应的转录因子T-bet/Gata-3/RORγt/Foxp3mRNA的表达量,采用试剂盒检测血浆中的超氧化物歧化酶(SOD)活性及丙二醛(MDA)浓度,并评价胃溃疡指数(UI)及制作病理切片。结果与N组比,S组UI加重,血浆MDA含量增高,SOD活性降低(P<0.05),Gata-3mRNA表达上调(P<0.05);RORγt与Foxp3mRNA表达均下调(P<0.05),而T-bet/Gata-3比值下降(P<0.05)。与S组比,P组UI减轻,血浆MDA含量降低,SOD活性升高(P<0.05),Gata-3mRNA表达下调(P<0.05),Foxp3mRNA表达上调(P<0.05)。结论应激性胃溃疡的发生发展可能与Th1/Th2细胞和Th17/Treg细胞失衡有关。凯时具有抗氧化作用,能减少应激性胃溃疡的发生,且在急性应激早期可能有保护机体免疫功能的作用。Objective To study the effects of lipo-PGEl on the Thl/Th2 and Thl7/Treg cell balance in stress-induced ulcer. Methods Forty male Sprague Dawley rats were randomly divided into the control group (group N), model group (group S), lipid microspheres 10μg/kg group (group L), lipo-PGE1 1 μg/kg group (group M) and lipo-PGE1 10 μg/ kg group (group P). After pretreatment, rats were subjected to water-immersion and restraint stress. Later, peripheral blood was obtained and lymphocyte and plasma was separated. The mRNA of specific transcription factors T-bet/Gata-3/ RORγt/Foxp3 which represented Thl/Th2/Thl7/Treg cell, were detected with RT-PCR. The activity of superoxide dismutase (SOD) and the levels of malondialdehyde (MDA) were assayed. Besides, ulcer index (UI) assessment and pathological sections of the stomach were carried out. Results Compared with group N, the UI and the levels of MDA of group S were higher, while the activity of SOD was lower (P〈0.05). The mRNA of Gata-3 was up regulated while the mRNA of RORγ/t and Foxp3 was down regulated (P〈0.05). Compared with group S, the UI and the levels of MDA of group P were lower, while the activity of SOD was higher (P〈0.05). The mRNA of Gata-3 was down-regulated while the mRNA of Foxp3 was up-regulated (P〈0.05). Conclusion The development of stress-induced ulcer may be related to the imbalances of Thl/Th2 cells and Thl7/Treg cells. Lipo-PGE1, an antioxidant, could reduce the occurrence of stress ulcer. It might protect the immune function at the early acute stress phase.
