摘 要:目的应用液基薄层细胞学和PCR HPV-DNA方法筛查宫颈癌前病变及早期宫颈癌。方法选取在门诊妇科要求宫颈癌筛查的患者2660例,同时行HPV-DNA检测和TCT检查,其中任何一项阳性者行阴道镜组织学活检。结果 2660例检查者,TCT阳性135例,HPV阳性344例。TCT和HPV均为阳性者为49例,阳性检出率分别为5.1%、12.9%和2.8%。TCT(+)135例行阴道镜下活检发现CINⅠ~Ⅲ53例,2例鳞状细胞癌。HPV(+)344例行阴道镜下活检CINⅠ~Ⅲ181例,鳞状细胞癌1例,TCT(+)和HPV(+)共49例,行阴道镜下活检发现CINⅠ~Ⅲ42例,有2例鳞状细胞癌,活检异常率分别为40.7%、52.9%和89.8%。结论 TCT联合HPV-DNA检测可提高宫颈癌前病变的诊断符合率。Objective Application of liquid-based cytology and PCR HPV-DNA method for screening cervical precancerous lesions and cervical cancer. Methods Selected in 2660 patients our hospital outpatient gynecologic requirements of cervical cancer screening, and HPV-DNA detection and TCT examination, any of which positive colposcopy biopsy. Results In 2660 cases, TCT positive 135 cases, HPV was positive in 344 cases. TCT and HPV were positive in 49 cases, the positive rates were 5.1%, 12.9% and 2.8%. TCT ( + ) 135 routine biopsy under colposcopy found CIN Ⅰ~Ⅲ53 cases, 2 cases of squamous cell carcinoma. HPV ( + ) 344 routine biopsy under colposcopy CINⅠ~Ⅲ 181 cases, 1 cases of squamous cell carcinoma, TCT ( + ) and HPV ( + ) a total of 49 patients with biopsy under colposcopy, line CIN Ⅰ~Ⅲ42 cases, there were 2 cases of squamous cell carcinoma, biopsy of the abnormal rates were 40.7%, 52.9% and 89.8%. Conclusion TCT and HPV-DNA detection can improve cervical precancerous lesions diagnosis coincidence rate.
