卡介菌多糖核酸膀胱灌注防治膀胱癌术后复发的临床研究

摘　　要：目的评价卡介菌多糖核酸(BCG-PSN)膀胱灌注预防膀胱癌术后复发的疗效与安全性。方法将74例患者随机分为BCG-PSN组38例和吡柔比星(THP)组36例。两组定期进行膀胱灌注,随访12-36个月,观察复发情况及不良反应。结果 BCG-PSN组膀胱癌术后复发率为18.4%(7/38),出现不良反应9例(23.7%);THP组复发率为19.4%(7/36),出现不良反应24例(66.7%)。两组复发率比较,差异无统计学意义(P>0.05),BCG-PSN组不良反应明显低于THP组,差异有统计学意义(P<0.01)。结论 BCG-PSN及THP都是有效预防膀胱癌术后复发的药物,但BCG-PSN膀胱灌注后不良反应明显小于THP组,是理想的膀胱灌注治疗药物。Objective To evaluate the curative effects and safety of intravesical instillation of Bacille Calmette-Guerin-polysaccharide nucleic acid （BCG-PSN） for preventing postoperative recurrence of bladder cancer. Methods 74 postoperative patients with bladder cancer were divided into two groups, the BCG-PSN group （38 cases） and the pirarubicin （THP） group （36 cases）. Tile two groups receivd intravesical instillation,once a week for 6 weeks, then once a month to one year. All the patients were followed up for 12-36 months,the tumor recurrence rate and the adverse reactions associated with therapy were observed. Results The tumor recurrence rate in the BCG-PSN group was 18.4% （7/38）, 9 cases （23.7%） appeared the adverse reactions, which in the THP group was 19.4% （7/36）, 24 cases （66.7%） appeared the adverse reactions. The tumor recurrence rates had no statistical difference between the two groups （P〉0.05）. But the adverse reactions in the BCG-PSN group were significantly lower than those in the THP group with statistical difference （P〈0.01）. Conclusion Intravesieal instillation of BCG-PSN and THP are effective to prevent bladder cancer postoperative recurrence,but the adverse reactions of BCG-PSN are significantly lower than those of THP, BCG-PSN is an ideal treatment drug for intravesical instillation.
