重性精神障碍患者对社区精神卫生服务的认知和需求研究

摘　　要：目的初步探讨重性精神障碍患者对目前社区精神卫生服务的认知和需求情况。方法按随机数字表法在2个三级甲等精神专科医院和4个区县所属社区抽取92例心境障碍患者和100例精神分裂症患者,用"社区精神卫生服务的认知和需求情况调查问卷"对其进行调查。结果 (1)重性精神障碍患者中完全不知道有社区精神卫生服务者为43.4%(82/189);(2)目前较为规律地接受了社区卫生服务的患者占37.6%(56/149);(3)对目前社区精神卫生服务非常满意和比较满意者仅分别占20.0%(36/180)和17.8%(32/180);(4)认为非常需要和比较需要社区精神卫生服务者分别占31.6%(60/190)和36.9%(70/190);(5)非常愿意和比较愿意参加社区精神卫生服务者分别占35.1%(67/191)和25.1%(48/191)。结论重性精神障碍患者对目前社区精神卫生服务的知晓率和认同度较低,其远不能满足患者的需求,积极建立和发展社区精神卫生服务来预防控制精神障碍应成为我国医疗卫生体制改革中的重点工作之一。Objective To explore the cognition and need for community mental health services in patients with severe mental disordersMethods We randomly selected 92 patients with mood disorder and 100 patients with schizophrenia from four communities and two large psychiatric hospitals in Beijing. The subjects were surveyed with the questionnaire on cognition and need for community mental health services. Results About 43.4% of the subjects were not aware of the availability of communi- ty mental health services. Only 37. 6% of the subjects had been provided with community mental health services regularly. Sub- jects who were very satisfied and satisfied with the current community mental health services accounted for 20. 0% and 17.8% of the subjects, respectively. Subjects who believed they greatly needed and needed community mental health services accounted for 31.6% and 36. 9%, respectively. Subjects who were quite willing to and willing to receive community mental health services ac- counted for 35. 1% and 25.1%, respectively. Conclusion Patients with severe mental health disorder have low cognition and need for community mental health services. The current community mental health services are not able to meet the needs of the pa- tients. Reform efforts should be made to utilize community mental health services in the prevention and management of mental dis- orders.
