可回收覆膜支架治疗贲门失弛缓症7例临床分析

摘　　要：目的观察可回收覆膜支架治疗贲门失弛缓症的疗效。方法选择贲门失弛缓症患者7例,其中男性2例,女性5例。在无痛下插入胃镜,放入导丝,退镜并交换导丝,观察贲门上缘距门齿距离,退出胃镜,选择直径20mm,长度8mm全覆膜支架,沿导丝推送支架至预定位置后释放支架。进镜观察支架位置正确后退镜。术后1周症状评分,1个月后取出支架。结果 7例均1次置入成功,1周内吞咽困难评分均降至0～1分,2例(28.6%)轻微胸痛,1例(14.3%)1周后支架脱落,重新置入成功。1个月后支架均成功取出,部分病例随访3个月无复发。结论可回收覆膜支架治疗贲门失弛缓症安全,疗效满意,并发症少,复发率低,值得临床推广。Objective To observe the effect of Recyclable coated stent treating esophageal achalasia.Methods 7 cases of esophageal achalasia patients were examined.There were 2 male cases and 5 female cases.After inserting gastroscope under Painless,the guide wire was placed.Pulled out gastroscope and exchanged guidewire.Observed the distance from the cardia upper edge to incisor,then Pulled out gastroscope.Selected the diameter of 20mm,length 8mm full coated stent,and Released it after pushing sent to a predetermined position along the guide wire.inserted gastroscope once again to check The correct position of Released sent,then Pulled out gastroscope.scored the symptoms after one week,and Removed sent after one month of operation.Results 7 cases were placed sents successfully.dysphagia scores dropped to 0-1 in the first week,and 2 cases had slight chest pain（28.6%）,and 1 case of sent was Slided which was replaced successfully.one month later,all of sents were taken out successfully.In some cases followed up for 3 months without recurrence.Conclusion Recyclable coated stent in the treatment of esophageal achalasia was safety,effective,Less complications,low recurrence rate,and it was worthy of clinical promotion.
