胸腹腔镜联合食管癌根治术治疗食管癌的疗效观察

摘　　要：目的探讨和评价胸腹腔镜联合食管癌根治术治疗食管癌的临床应用价值及其安全性。方法回顾性分析2010年4月—2012年4月由同一组医师完成的60例行胸腹腔镜联合食管癌根治术的食管癌患者,其中30例食管癌患者(A组)行胸腹腔镜联合食管癌根治术,另30例食管癌患者(B组)行传统的外科手术。比较2组患者的手术时间、术中出血量、淋巴结清扫数目、术后并发症情况、术后肛门排气时间以及住院时间指标。结果与传统的手术组相比,行胸腹腔镜联合食管癌根治术的患者的手术时间明显升高,但术中出血量、住院时间、术后肛门排气时间、术后并发症率下降明显,淋巴结清除数目无统计学差异。结论胸腹腔镜联合食管癌根治术治疗食管癌具有出血少、恢复快、手术安全、微创等优点,值得临床推广应用。Objective To discuss the curative effect of laparoscopy combined with emphagea[ cancer radical operation in the treatment of esophageal carcinoma. Methods Sixty pa- tients with laparoscopy combined with esophageal cancer radical operation from April 2011 to April 2012 were operated by the same group of physicians and analyzed retrospectively. The esophageal cancer patients （group A, n = 30 ） received thoracic laparoscopic combined with resection of esophageal cancer. The other patients （group B, n= 30） received the traditional surgical operation. The indices of operation time, intraoperative blood loss, number of lymph node dissection, postop- erative complications, postoperative anal exhaust time and hospitalization time were compared. Re- sults Compared with the traditional operation group, the operation time of the patients with la- paroscopy combined with esophageal cancer radical operation was significantly longer, but the intra- operative blood loss, hospital stay, postoperative flatulence, and postoperative complication rates de- creased significantly. The number of removed lymph nodes showed no statistically significant difference between the two groups. Conclusion Laparoscopy combined with esophageal cancer radical operation in the treatment of esophageal cancer has the advantages of less bleeding, safe, quick recov- ery, minimally invasive operation and other advantages. Thus it is worthy of clinical application.
