摘  要:目的探讨糖皮质激素(GC)对重症肌无力(MG)患者血清外周血白介素18(IL-18)和转化生长因子β-1(TGFβ-1)水平的干预作用。方法随机选取在我院治疗的52例重症肌无力患者为干预组,按照年龄和性别匹配60例健康对照组,采用ELISA检测干预组的患者糖皮质激素治疗前,治疗后血清IL-18,TGFβ-1水平以及健康对照组的血清IL-18,TGFβ-1水平,采用临床绝对和相对记分法判定临床疗效。结果干预组患者的血清IL-18,TGFβ-1水平与对照组差异显著(P<0.05);治疗后干预组血清IL-18水平明显下降(P<0.05),TGFβ-1水平明显升高(P<0.05)。结论糖皮质激素可以抑制IL-18,并且提高血清TGFβ-1水平进一步有效调节重症肌无力患者的细胞免疫功能。Objective To investigate the effect of glucocorticoid (GC) on the serum levels of interleukin-18 (IL-18) and transforming growth factorβ-1 (TGFβ-1) in new onset patients with myasthenia gravis (MG). Methods RandomLy selected 52 cases of patients with myasthenia gravis in our hospital for intervention group, according to age and gender matching 60 patients healthy controls. The serum level of IL-18 and TGFβ-1 were measured in 52 patients with MG before and after treatment with GC and in 60 health controls with ELISA. The absolute and relative scores of 52 patients with MG were rated by score system. Results The level of IL-18 and TGFβ- 1 of patients with MG were significantly higher or lower than those of health group. The serum levels of IL-18 were significantly decreased and the levels of TGFβ-1 were increased after 2 months treatment with GC. There was not correlation between variation of IL-18 or TGFβ-1 concentrations and the relative scores of MG pre-and post-treatment. Conclusion GC can effectively regulate the function of cellular immunity by inhibiting the production of IL-18 and increasing secretion of TGFβ-1.
