摘　　要：:目的　对胃癌淋巴转移进行分析。探讨合理切除淋巴范围。方法　通过 1498例胃癌淋巴转移资料进行综合分析 ,特别是胃癌Ⅰ、Ⅱ、Ⅲ站各组淋巴结转移情况。结果　发现进展期胃癌的Ⅰ、Ⅱ站淋巴结有较高的转移率 ,Ⅲ站仅部分组淋巴结有较高转移率。在此基础上 ,设计一种名叫选择性D3 的进展期胃癌扩大淋巴结切除手术。结论　通过廿余年临床实践 ,选择性D3 手术能明显提高Ⅲ期和部分Ⅳ期胃癌的生存期。手术创伤小 ,可以避免一部分不必要的全胃切除 。Objective To study the rule of lymphatic metastasis of progressive gastric carcinoma aiming at the proper scope of clearance of lymph nodes.Methods The resected lymph nodes of different stations of every case of gastric carcinoma in a group of 1498 cases were carefully studies, with special reference of lymph nodes of station 1, 2 and 3. Results The lymph nodes of both stations 12 and part of station 3 had high rate of metastasis. An improved radical operation (called selective D3 operation )was designed on the basis of this detailed study. Conclusion Through more than 20 years of practice, statistical results showed this new operation significantly prolonged the survival time in stage Ⅲ gastric carcinoma and part of stage Ⅳ cases. The operative injury was less severe and the unnecessary total gastrectomy could be avoided in selective cases, hence the postoperative performence states of patients was much improved.
