摘　　要：目的 :评价 MRI对髓外硬膜下肿瘤的诊断价值。方法 :经手术、病理证实的髓外硬膜下肿瘤 6 9例、83个瘤灶 (其中 1例转移癌为随访证实 ) ,回顾性分析其 MRI表现。结果 :( 1)神经源性肿瘤多为 T1 WI低信号、T2 WI高信号 ,T1 / T2 得分比值为 0 .31± 0 .2 ,信号不均匀 ,可见肿瘤血管 ,增强扫描强化明显 ,但不均匀 ;( 2 )脊膜瘤 T1 WI多为等信号或稍低信号 ,T2 WI多为等或稍高信号 ,T1 / T2 得分比值为 0 .85± 0 .19,信号较均匀 ,增强扫描强化均匀 ,增强效果不如神经源性肿瘤高 ,可见脊膜尾征 ;上述肿瘤的 T1 、T2 、T1 / T2 得分比值及强化效果经统计学处理差异有显著性 ;( 3)其它 :脂肪瘤 MRI表现具有特征性 ;转移癌 MRI表现可类似脊膜瘤。结论 :髓外硬膜下肿瘤的 MRI诊断价值极高 ,定位诊断正确率为 98.80 % ,定性诊断正确率为 95 .2 %。Objective:To evaluate MRI diagnostic value of intradural extramadullary neoplasm.Methods:The MRI appearances of 69 cases(83 lesons) verified by surgery and pathology(but one)were retrospectively analysed.Results:(1)neurinomas displayed commonly low-signalintensity on T 1WI,high-signal-intensity on T 2WI,T 1/T 2=0.31±0.2,the signal was heterogeneous.The vessels of tumors could be founed;(2)The MRI characteristics of meningioma were iso-signal or slightly low-signal on T 1WI,iso-signal or slightly high-signal on T 2WI,T 1/T 2=0.82±0.19,and the signal was more homogeneous.Signal intensity of T 1WI-GD-DTPA was lower than that of neurinomas;Appearances of T 1,T 2,T 1/T 2 and enhancement of above mentioned tumors were of notable differences statistically;(3)The MRI findings of others:lipomas had characteristic appearance;the MRI appearances of metastas were not characteristic.Conclusion:The diagnostic value of intradural extramedullary tumor by MRI is unique,the accurate rate in located and qualitative diagnosis are 98.80% and 95.2% respectively.
