摘 要:目的:探讨抑郁症患者颅脑平扫CT值与增强扫描后强化程度的特征,为研究抑郁症发病机制与诊断提供客观数据基础。方法:随机选择符合《中国精神疾病分类方案与诊断标准(第3版)》中抑郁症诊断标准的20例患者作为研究组,选择性别年龄相匹配的健康人20例作为对照组。2组分别行头颅CT平扫与增强扫描,比较各脑叶的平扫CT值及增强扫描后CT值递增峰值。结果:①抑郁组大脑左、右额叶脑组织密度CT值分别为(31.56±1.48)HU、(30.65±1.55)HU,明显低于对照组的(34.52±1.36)HU、(34.78±1.86)HU(P均<0.01)。②抑郁组大脑左、右额叶脑组织密度增强扫描后CT值递增峰值分别为(4.68±0.62)HU、(4.51±0.53)HU,明显低于对照组的(9.52±0.66)HU、(9.69±0.56)HU(P均<0.01)。结论:抑郁症患者大脑左右额叶脑组织密度CT值较正常人低,增强扫描后强化程度较正常人低,这可能是大脑额叶功能损害的神经解剖学基础,脑血流量灌注减少,使得大脑额叶功能改变。Objective: To investigate the characteristic of the plain and enhanced CT value of brian in patients with depressive disorder,in order to provide objective data base for the the pathogenesis and diagnosis of depression patients. Methods: 20 pa-tients consistent with the criteria of CCMD-3 depressive disorder were concluded as the study group,and based on sex and age matched healthy people were selected as the control group. Two groups were measured with multi-slice spiral head CT with plain and enhanced scanning respectively,and measured the CT value in different interesting region of the lobes. The CT values were compared between each of them. Results:The CT values of the study group in left and the right frontal lobe were(31.56±1.48) and(30. 65 ± 1.55) respectively, it was significantly lower than that of the control group (34.52±1.36 ) and ( 34.78±1.86), ( P〈0. 01). Post contrast the increased CT value in left and the right frontal lobe of brain in study group were(4.68±0. 62)and(4. 51±0. 53)respectively, it was significantly lower than that of the control group(9.52±0.66)and(9.69 ±0.56), ( P〈0.01). Conclusion: Both of the plain and enhanced CT value of the left and right frontal lobe of brain tissue in study group is lower than those of control group,which might be responsible for the neuroanatomical basis of frontal lobe dysfunction and the decreasing of cerebral blood flow perfusion makes the function change of frontal lobe.
