摘　　要：目的 探讨3．0TMRI普通平扫联合DWI对乳腺癌筛查的临床价值。方法 收集本院行乳腺MRI检查并行穿刺或手术病理证实的乳腺病变患者60例，比较普通平扫、普通平扫联合DWI与动态增强检查诊断乳腺癌的灵敏度、特异度。结 果普通平扫、普通平扫联合DWI以及增强检查的灵敏度和特异度分别为57．44％、84．62％、91．48％和76．92％、97．87％、38．46％。ADC值为1．1×10^-3mm^2／s为乳腺癌的临界值。普通平扫对乳腺癌的例数及病灶总数检出率均低于动念增强扫描（P〈0．0125）。普通平扫联合DWI对乳腺癌的例数及病灶数检出率与增强扫描无显著差异（P=0．267，P=0．804）。结论普通平扫联合DWI定性及ADC定量对病灶进行分析，可提高对乳腺癌检出的准确性。3.0T MRI普通平扫联合DWI对乳腺癌的筛查具有较高的临床价值。Objective To explore the value of 3.0T general non-enhancement MRI combined with DWI in screening of breast cancer. Methods MRI imagines were obtained from 60 patients in our hospital whose pathologies were confirmed by biopsy or sur gery. Two experienced radiologists read the images blindly and compared the sensitivity and specificity of diagnosis of breast cancer with the general non-enhancement MRI, the general non-enhancement MRI combined with DWI and the enhancement MRI. Results The sensitivity and specificity of the general non-enhancement MRI, the general non-enhancemenl MRI with DWI and the enhance ment MRI were 57. 44% ,84. 62%, 91. 48% and 76. 92%, 97.87% , 38. 46% respectively. There was a significant different between the general non-enhancement MRI and the enhancement MRI （P〈0. 0125）. While there was no significant different be- tween the general non enhancement MRI combined with DWI and the enhancement MRI （P= 0. 267, P=0. 8041. Conclusion DWI and ADC can qualitatively and quantitatively analysis the lesion, improve the accuracy of detection of breast cancer. Hence, the gen- eral non enhancement MRI combined with DWI may play an important role in breast cancer screening.
