消化道异位胰腺的CT误诊分析

摘　　要：目的 分析22例消化道异位胰腺的CT表现，分析其误诊及漏诊原因，提高对该病的认识。方法 回顾性分析22例经手术及病理让实的异位胰腺的CT征象。结果22例异位胰腺中，胃大弯2例，胃小弯4例，胃窦9例，十二指肠3例，空肠2例，回肠2例；黏膜下层8例，肌层8例，浆膜层3例、浆膜外3例；22例行CT平扫，13例行CT增强检查，6例明显强化，3例呈中等强化，4例强化不明显；13例中2例诊断为异位胰腺，6例诊断为间质瘤，1例诊断为囊肿，1例学虑为胃癌，2例诊断为淋巴结，9例因合并其他疾病而漏诊。结论异位胰腺在CT中表现复杂，但存在特征性改变，结合临床资料，做出全面分析。可以全面提高异位胰腺的诊断率，指导临床手术方式。Ohjeclive To review the CT findings in 22 cases with gastrointestinal ectopic pancreas and analysis the reason of ntisdiagnosis and missed diagnosis, which subsequently improve our understanding of the disease. Methods CT findings were reviewed retrospectively in 22 cases with ectopic pancreas confirmed by surgery and pathology. Results Among the 92 cases with ectopic pan creas, the locations of pancreas included 2 cases in greater curvature,4 in the lesser curvature,9 in the gastric antrum,3 in the duode num,2 in the jejunum of cases ,2 in the ileum,8 insubmucosa,8 inmuscularis,3 in serosa and 3 outside theserosa. All patients were examined by plain CT scan and 13 patients received enhanced CT scan. Thirteen of 22 patients were found abnormal and the diagno- sis varied including 2 cases as eetopic pancreas,6 as stromal tumor, I as cyst , 1 as gastric cancer and 2 were diagnosed as lymph node. The other 9 cases with other diseases were miss diagnosis of ectopic pancreas. Conclusion CT manifestations of ectopic pancreas are complex, but characteristic. Making a comprehensive analysis by combining CT and clinical data may improve the diagnosisand direct the operation mode.
