原发性中枢神经系统淋巴瘤16例临床病理特征及免疫表型分析

摘　　要：目的探讨原发性中枢神经系统淋巴瘤(primary central nervous system lymphoma,PCNSL)的临床病理特征及免疫表型。方法对2007年4月～2012年5月昆明医科大学第一附属医院诊治的16例PCNSL患者的临床病理资料进行回顾性分析。采用免疫组化SABC法对其免疫表型进行分析。结果 16例PCNSL患者年龄7～74岁,平均52.3岁,男女比例为2.2∶1。额叶和枕叶为PCNSL好发部位。镜下见肿瘤细胞弥漫浸润性分布,多聚集在血管周围,呈袖套样结构。免疫表型:16例PC-NSL中CD45均呈阳性,15例CD20呈阳性,12例CD79a呈阳性,13例MUM1呈阳性,GFAP均呈阴性,表明16例均为B细胞淋巴瘤。弥漫性大B细胞淋巴瘤是PCNSL的主要病理类型(16例PCNSL中11例为弥漫性大B细胞淋巴瘤)。结论 PCNSL的主要病理类型为弥漫性大B细胞性淋巴瘤,病理学检查和免疫表型分析是其确诊的金标准。Purpose To study the clinicopathological characteristics and immunophenotypes of primary central nervous system lympho- ma （PCNSL）. Methods The clinicopathological data of 16 histopathologically proven cases of PCNSL were retrospectively analyzed from April 2007 to May 2012. lmmunophenotypes were analyzed by immunohistochemistry. Results The age of 16 PCNSL patients ranged from 7 to 74 years with an average age of 52. 3 years. The ratio of male to female was 2.2 ： 1. Frontal lobe and occipital lobe were the most common sites of involvement. Diffuse distribution and perivascular arrangement in a sleevelet-like pattern of the tumor cells were the pathological characteristics of PCNSL. lmmunohistochemical resuhs showed CD45 （ ＋ ） and GFAP（ - ） in all cases, CD20（ ＋ ） in 15 cases, CD79a（ ＋ ） in 12 cases and MUM1 （ ＋ ） in 13 cases, suggesting B lymphocyte origin of all cases. Diffuse large B-cell lymphoma （ 11 cases in 16 cases of PCNSL） was the major histological pattern of PCNSL. Conclusion Diffuse large B- cell lymphoma is the major pathological type of PCNSL, pathological examination and immunophenotyping are the gold standard for di- agnosing PCNSL.
