双重氧化脱黑色素法在恶性黑色素瘤免疫组化诊断中的应用

摘　　要：目的:探讨双重氧化脱黑色素法在恶性黑色素瘤免疫组化诊断中的应用。方法:20例经病理确诊为恶性黑色素瘤的石蜡组织标本,用5种不同浓度的过氧化氢液先氧化脱色,经观察选取15%的过氧化氢液氧化30min后,再用0.5%高锰酸钾液加强氧化5min,最后5%草酸液清洁5min,应用Envision免疫组化法检测Melan-A、Melanoma、S-100和VIM阳性的表达。结果:20例恶性黑色素瘤切片在脱色素前HE染色可见棕黄色色素颗粒,经双重氧化脱色素处理后,HE染色效果好,无脱片;应用15%的过氧化氢脱色效果优于其他4种浓度的过氧化氢,滴加0.5%高锰酸钾5min,再滴加5%草酸液5min,黑色素完全消失;脱色处理后Melan-A、Melanoma、S-100和VIM阳性定位准确,着色好。结论:采用双重氧化脱黑色素法进行免疫组化染色具有较理想的作用,值得采用。OBJECTIVE： To investigate the removal method of double oxidation of melanin in immunohistoehemical diagnosis of malignant melanoma. METHODS： The paraffin samples from 20 patients diagnosed as malignant melanoma were treated with five different concentrations of hydrogen peroxide bleaching solution before oxidation, After 30 minutes 15 % hydrogen peroxide solution were selected and treated with 0.5 % potassium permanganate solution to enhance oxida tion of 5 minutes and 5 % oxalic acid cleaning solution was used for 5 minutes,Envision immunohistochemical assay was used to detect Melan-A, Melanoma, S-100andVIM expression. RESULTS： The demelanin efficacy was better in 15 % of the oxidation of hydrogen peroxide solution for 30 minutes than other concentrations, Adding 0.5 % potassium permanganate solution for 5 min and 5 % oxalic acid for 5 rain could enhance oxidation and completely remove the melanin. CONCLU- SION： Double oxidation of de-melanin in immunohistochemical staining is a recommendable method in diagnosis of mela noma.
