混合能量电子束全身照射技术的临床应用及特点

摘  要:目的比较4MeV或6MeV单能射线和4MeV、6MeV混合能量多野全身电子线照射技术的相关物理参数、放射物理学和临床治疗特点。方法使用三维水箱、仿真人体模、剂量仪等专业测量设备分别对Elektra Precise加速器4MeV或6MeV单能射线和4MeV、6MeV混合能量的物理和临床参数进行实际测量,经过对实际测量结果的归纳、分析、比较,得到可以满足全身电子线照射技术(TSEI)治疗蕈样霉菌病患者的物理和临床参数的最佳能量配置。结果在增加5mm有机玻璃散射屏的情况下,4MeV和6MeV混合能量照射时实际测量得到的物理参数可满足临床要求。结论混合能量射线照射,通过改变能量和混合的权重,即可获得不同的PDD参数,可以满足临床治疗的要求;而且与单能电子线治疗相比能够缩短治疗时间。Objective To compare the characteristics of radiophysics of the muti-fields full body irradiation by 4 Mev and 6 Mev mixed-energy electron beam with the 4 Mev or 6 Mev single-energy electron beam. Methods 3D- water tank, anthropomorphic phantom and dosimeter were used to measure the physical and clinical parameters of 4 Mev or 6 Mev single-energy electron beam or 4 Mev and 6 Mev mixed-energy electron beams generated by Elektra Precise Accelerator, respectively. In addition, the actual measurement results were summarized, analyzed and compared with each other so as to work out the optimal energy allocation of physical and clinical parameters for treating mycosis fungoides. Results When a 5 mm plexiglass scattering screen was applied, the actually measured physical parameters of the 4 Mev and 6 Mev mixed-energy electron beam could meet the clinical requirements. Conclusion The mixed-energy irradiation can not only meet the clinical treatment requirements, but also shorten the treatment time compared to the single-energy electron beam radiation treatment.
