原发性食管燕麦细胞癌——附21例报告

摘　　要：原发性食管燕麦细胞癌少见。其生物特性为恶性程度高,易早期扩散转移。本文报告了我院于1979年4月～1990年3月间发现的21例(21/2121)。其中,20例为“纯”燕麦细胞癌,1例同时合并鳞癌。文章对其病理特征、诊断以及治疗等问题作了重点讨论。预后较食管鳞癌差。单一手术者大多数在1年内死亡;综合治疗中2例生存超过3年以上。作者强调:早期确诊;尽早根治性切除;综合治疗是提高本病疗效的关键。Primary oat cell carcinoma of the esophagus is a rare malignant tumor which has a distinct biologic. behavior with early invasion and metastases. 21 such patients (21/2121) treated in our hospital from 1979.4-1990.3.were reported Of them, 20 were 'pure' oat cell carcinomas, while 1 was typical oat cell with squamous cell carcinoma. The pathologic feature, diagnosis and management of the disease were emphatically discussed. The prognosis of this tumor was worse than that of the squamous carcinoma of the esophagus. Most of the patients treated only by operation died within 1 year. Two patients treated by comprehensive therapy are alive and well for more than 3 years. The authors believe that the the survival rate of the disease may be increased by early diagnosis and early radical resection with comprehensive adjuvant therapy.
