食管鳞癌热休克蛋白70表达与放疗疗效关系的研究

摘　　要：目的研究食管鳞癌中休克蛋白70（HSP70）的表达与放疗敏感性的关系。方法收集2002年1月至2007年12月在郑州人民医院手术的食管鳞癌患者的新鲜标本组织共70例，分析HSPT0表达情况与放疗效果的关系，制备抑制HSPT0表达的siRNA质粒，培养并转染质粒到食管癌EC9706细胞系，1射线照射后比较与对照组细胞的数量，用Western印迹了解转染质粒细胞照射后HSP70的表达情况。结果HSP阳性化疗或化放疗组患者，2年复发率低于单纯放疗组（35％比55％，P〈0．05），HSP阴性患者无区别；^y射线照射后，RNA干扰组的细胞系数量明显少于RNA表达组（2Gy：70％比80％，4Gy：55％比75％，6Gy：30％比50％，8Gy：15％比33％，10Gy：3％比10％），Western印迹提示：1射线照射后，HSP70表达上升，16h达到峰值，20h开始下降。结论HSPT0的高表达预示对放疗不敏感，干扰HSP70的合成可能成为提高食管癌放疗疗效的新方法。Objective To explore the relationship between heat shock protein 70 （HSP70） expression and radiotherapeutic sensitivity. Methods Retrospective analysis was conducted for HSP70 expression in surgical specimens of 70 patients with esophageal cancer and their radiotherapy effect; the siRNA plasmids were prepared to inhibit HSP70 expression. After culturing, the plasmids were transfected into EC9706 cells. After ~/-ray irradiation, the cell numbers were compared with the control group. And the contents of HSP70 protein were measured by Western blot in transfected cells. Results In HSP positive group, patients with chemotherapy or chemoradiotherapy had a lower 2-year recurrence rate than the radiotherapy group（35% vs 55% ）. In HSP-negative group, there was no significant difference. After y-ray irradiation, the number of RNA interference cell lines was significantly less than that of the RNA expression cell lines. Western blot showed that, after y-ray irradiation, HSP70 expression increased, peaked at 16 hours and began to decline at 20 hours. Conclusion Esophageal squamous cell carcinoma is not sensitive to radiotherapy. The interference of HSP70 protein synthesis may become a new method of improving the efficacy of radiotherapy.
