三阴性乳腺癌组织中Ki-67、p53、CK5/6的表达及预后分析

摘　　要：目的探讨三阴性乳腺癌患者的临床病理特点及预后。方法回顾性分析122例三阴性乳腺癌患者及100例非三阴性乳腺癌患者的临床资料,对所有石蜡标本行Ki-67、p53、CK5/6表达的染色,并随访统计其复发转移率和生存率。结果 TNBC组的Ki-67、p53、CK5/6的阳性表达率明显高于NTNBC组(P<0.05或P<0.01)。TNBC组获得5.5(0.6～10)年随访,NTNBC组获得5.9(0.2～10)年随访。TNBC组有35例(28.7%)出现局部复发或远处转移,NTNBC组有15例(15%)出现复发转移,TNBC组的局部复发或远处转移率明显高于NTNBC组(P<0.05)。5年生存率TNBC组为78.5%,NTNBC组为86.7%,TNBC组的总生存率明显低于NTNBC组(Log rank=5.80,P<0.05)。结论与NTNBC相比,TNBC中Ki-67、p53、CK5/6的阳性表达率相对较高,而其5年生存率相对较低。进一步研究其分子生物学靶点,可能为寻求新的治疗手段提供帮助。Objective To study the clinical pathological characteristics and prognosis in patients with triple negative breast cancer （TNBC）. Methods A retrospective analysis was carried out in 122 cases of TNBC and lO0 cases of non-negative breast cancer patients （NTNBC）. All paraffin specimens were stained of Ki-67, io53, CK5/6 monoclone antibodies by ABC immunohistochemical method. And it was also analyzed of their recurrence or metastasis rates and survival rates. Results The positive expression rates of Ki-67, IO53, CK.5/6 were significantly higher in TNBC group than those in NTNBC group （ P 〈 0.05 or P 〈 0.01 ）. TNBC group was followed up for 5.5 （0.6 - 10） years, while NTNBC group was followed up for 5.9 （0.2 ～ 10） years. In the TNBC group, there were 35 cases （28.7%） occurred local recurrence or metastasis, and only 15 （15%） occurred in the NTNBC group （P 〈0. 05）. The 5-year survival rate was 78.5% in TNBC group, which was lower than 86.7% of NTNBC group （ Log rank = 5.80, P 〈 0.05 ）. Conclusions As compared to NTNBC patients, the Ki-67, 1053 CK5/6 positive expression rates in TNBC is relatively high, whereas their 5- year survival rate is relatively low. Further study of the molecular biological target may provide assistance to seek new approaches to the TNBC treatment.
