摘 要:目的评价乳腺局灶性病变的超声乳腺影像和报告资料系统(BI-RADS-US)分级及其对良、恶性病变的鉴别价值。方法回顾性分析经病理证实的315例乳腺局灶性病变(其中良性病变223例,恶性病变92例)的超声BI-RADS-US分级,比较分析BI-RADS-US分级与病理结果,统计BI-RADS-US诊断乳腺局灶性病变的特异性、敏感性、准确性、阳性及阴性预测值。结果 BI-RADS-US分级2级13例,3级97例,4级153例,5级52例;2~5级诊断恶性肿瘤的阳性预测值分别为0、2.1%、25.5%及98.1%。BI-RADS-US诊断乳腺恶性肿瘤的特异性、敏感性、准确性、阳性及阴性预测值分别为48.4%、97.8%、62.9%、43.9%及98.2%。结论 BI-RADS-US分级对预测乳腺良、恶性病变有较高的可信度。Objective To categorize breast lesions according to sonographic BI-RADS lexicon and to evaluate the reliability of BI-RADS in differentiating benign from malignant breast lesions. Methods Three hundred and fifteen breast masses confirmed by pathology were classified according to BI-RADS. Pathologic results were compared with BI-RADS. The specificity, sensitivity, accuracy, positive predictive value (PPV) and negative predictive value (NPV) of the sonographic BI-RADS lexicon were calculated. Results Of 315 cases, 223 of them were benign lesions, 92 of them were malignant lesions and the prevalence of breast cancer was 29.2%. Thirteen masses were classified to grade 2, 97 to grade 3, 153 to grade 4, and 52 to grade 5. The positive predictive values of five grades were 0, 2.1%, 25.5% and 98.1%, respectively. The specificity, sensitivity, accuracy, PPVand NPV of sonographic BI-RADS lexicon for the diagnosis of malignant breast tumor were 48.4%, 97.8%, 62.9%, 43.9% and 98.2%, respectively. Conclusion The sonographic BI-RADS lexicon is reliable for the differentiation of benign and malignant breast tumor.
