经阴道彩色多普勒能量图结合CA_(125)诊断卵巢子宫内膜异位囊肿的价值

摘　　要：探讨经阴道彩色多普勒能量图（ＣＤＥ）结合糖类抗原１２５（ ＣＡ１２５）诊断卵巢子宫内膜异位囊肿的价值。方法：对１６０例卵巢肿块患者于术前７ｄ内进行经阴道彩色多普勒能量图检查及血清ＣＡ_１２５检查，然后与术后病理对照。实验结果与病理结果符合程度用Ｋａｐｐａ值统计。结果：经阴道彩色多普勒能量图诊断卵巢子宫内膜异位囊肿与病理符合程度好，ｋａｐｐａ值０．８８。而结合ＣＡ_１２５诊断该病Ｋａｐｐａ值较低为０．６４。结论：独自应用经阴道彩色多普勒能量图诊断该病是术前诊断方法中有效的手段之—。Objective:To evaluate the value of transvaginal Color Doppler Energy combined with CA_125 in the differential diagnosis of endometriotic cysts. Method: Transvaginal Color Doppler Energy examination and measurement of CA_125 plasma level were performed in 160 patients with ovarian mass within 7 days before surgery. The ultrasongraphic imprssion and CA_125 value were compared with the histopathologic diagnosis.The overall agreement between the test result and the actual outcome was calculated using the kappa statistic Results:Transvagin al Color Doppler Energy had a strong agreement between test result and pathology,kappa value 0.88,whereas the combined method was associated with a lower kappa value 0.64 .Conclusion: Transvagin al Color Doppler Energy used alone is one of the most cost-effective methods in the preoperative differential .diagnosis of endometriotic cysts.
