摘  要:目的 观察 nm2 3H1 表达与妊娠滋养细胞肿瘤转移及细胞增殖状态的关系 ,并观察化疗对滋养细胞肿瘤细胞增殖状态的影响。方法 采用免疫组化方法检测 48例妊娠滋养细胞肿瘤手术标本、10例完全性葡萄胎和10例正常早孕绒毛组织刮宫标本中 nm2 3H1 和 PCNA的表达。结果 正常早孕绒毛或葡萄胎的 nm2 3H1 表达较妊娠滋养细胞肿瘤强 (P<0 .0 1) ,妊娠滋养细胞肿瘤中侵蚀性葡萄胎较绒癌强 (P<0 .0 5 ) ,WHO分期 、 期者较 、 期者强 (P<0 .0 5 ) ,WHO评分非高危组较高危组强 (P<0 .0 5 )。妊娠滋养细胞肿瘤 PCNA阳性细胞数高于正常早孕绒毛 (P<0 .0 5 )。结论  nm2 3H1 免疫组化检测对妊娠滋养细胞肿瘤转移和预后的评估以及化疗方案的选择具有参考价值。Objective To evaluate the relationship between nm23H 1 expression levels and metastasis and proliferation of gestational trophoblastic tumors(GTTs) and to observe the influence of chemotherapy on proliferative status of GTTs.Methods Expression of nm23H 1 and PCNA was determined in 48 cases of gestational trophoblastic tumors,10 complete hydatidiform mole and 10 normal first trimester trophoblast with immunohistochemical methods.Results The results show that nm23H 1 expression was stronger in first trimeter trophoblasts and hydatidiform mole than that in GTTs( P <0.01).nm23H 1 expression in invasive hydatidiform mole was stronger than that in choriocarcinoma( P <0.05);it in those of WHO stage I,II was stronger than that in stage Ⅲ,Ⅳ;and WHO scoring non high risk group had stronger nm23H 1 expression than high risk group.GTTs have more PCNA positive trophoblastic cells than normal first trimester trophoblast( P <0.05).Conclusion nm23H 1 detection has referential value in evaluating metastasis potential and prognosis of GTTs.
