摘　　要：目的评价齐拉西酮对精神分裂症患者生活质量的影响,为临床干预提供依据。方法将120例精神分裂症患者随机分为2组,每组60例,研究组口服齐拉西酮治疗,对照组口服利培酮治疗,观察8周。于治疗前及治疗8周末采用阳性与阴性症状量表(PANSS)、副反应量表(TESS)评定临床疗效及不良反应,用生活质量综合评定问卷-74(QGOLI-74)评定生活质量。对2组评定结果进行对比分析。结果治疗后2组PANSS总分及各因子分均较治疗前显著下降(P<0.01),QGOLI-74总分、躯体健康、心理健康、社会功能维度分及各因子分均较治疗前有显著提高(P<0.01);治疗8周末研究组QGOLI-74总分、躯体健康维度、社会功能维度、躯体不适、运动与感觉、工作与学习及业余娱乐等因子分均显著高于对照组(P<0.05或0.01);TESS评分及锥体外系反应、心电图异常、体重增加等不良反应发生率均显著低于对照组(P<0.05)。结论齐拉西酮治疗精神分裂症疗效显著,与利培酮相当,且安全性更高,更有利于提高患者的生活质量。[Objective]To evaluate the effect of ziprasidone on quality of life in schizophrenics,provide the basis for clinical intervention.[Methods]120 schizophrenics were randomly divided into two groups,60 cases in each group.The research group was given ziprasidone orally,the control group was given risperidone orally,for 8 weeks.The clinical effect and adverse reaction were assessed with the Positive and Negative Syndrome Scale（PANSS） and the Treatment Emergent Symptom Scale（TESS） before treatment and after 8 weeks treatment,the quality of life was evaluated with the Generic Quality of Life Inventory-74（QGOLI-74）.The results of two groups were contrastively analyzed.[Results]After treatment,the total score and factor scores of PANSS in two groups have reduced significantly as compared with those before treatment（P0.01）,while the total score,scores for physical health,mental health,social function and each factor of QGOLI-74 have increased significantly as compared with those before treatment（P0.01）.At the end of the 8th week,the total score and factor scores for physical health,social function,body discomfort,movement sensation,work learning and amateur recreation in the research group were significantly higher than those in the control group（P0.05 or P0.01）.TESS score and the incidence rates of adverse reactions,such as extrapyramidal system reactions,abnormal electrocardiogram and weight gain,were significantly lower than those in the control group（P0.05）.[Conclusion]Ziprasidone has an evident effect for schizophrenia,which is equivalent to risperidone and has higher safety,and it is more beneficial to improve patients＇ quality of life.
