多西他赛联合奥沙利铂和5-氟尿嘧啶诱导化疗对口腔鳞癌的疗效观察

摘　　要：目的观察多西他赛(TXT)联合奥沙利铂(L-OHP)和5-氟尿嘧啶(5-Fu)诱导化疗对口腔鳞癌的治疗疗效和毒副作用。方法 53例经病理组织学证实为口腔鳞癌的患者,给予多西他赛联合奥沙利铂和5-氟尿嘧啶治疗,TXT 75mg/m2,L-OHP 130mg/m2,5-Fu 500mg/m2静滴,21天为1个疗程,连用2个疗程后评定近期疗效,用药结束后1-2周对患者施行手术根治术,随访3-5年评定远期疗效。结果近期疗效中,完全缓解(CR)11例,部分缓解(PR)29例,无效(NR)13例,总有效率(CR+PR)为75.47%(40/53),未出现严重不良反应;远期疗效中,患者术后3、5年生存率分别为64.15%(34)、50.94%(27)。结论多西他赛联合奥沙利铂和5-氟尿嘧啶用于口腔鳞癌术前诱导化疗,疗效明显,毒副反应可以耐受,与手术治疗结合可提高患者生存率。【Objective】To observe the effect and adverse reaction of docetaxel combined with oxaliplatin and 5-fluorouracil for oral squamous cell carcinoma.【Methods】Fifty three patients confirmed to be oral squamous cell carcinoma by histopathology,received a course of induction chemotherapy（docetaxel 75mg/m2,oxaliplatin 130mg/m2,5-fluorouracil 500mg/m2）.After a cycle of 21 days,surgical operations were perfomed for these patients-Short-term curative effects were assessed and 3,5-year survival rates were followed up.【Resluts】In short-term effect,CR was seen in 11 cases,PR in 29 cases,NC in 13 cases,the total effective rate（CR ＋PR） was 75.47%（40/53）.Toxicity was minor and occurs did not affect the application of this therapy.The 3,5-year survival rates were 64.15%（34） and 50.94%（27）,respectively.【Conclusion】 The induction chemotherapy of docetaxel combined with oxaliplatin and fluorouracil is an effective and tolerable regimen in patients with oral squamous cell carcinoma,if followed by surgical intervention,it will improve the survival rate.
