顺铂胸腔内注射治疗食管癌术后乳糜胸

摘　　要：目的评价顺铂胸腔内注射治疗食管癌术后乳糜胸的临床应用价值。方法回顾分析了安阳市人民医院2004年7月至2011年6月食管癌术后8例乳糜胸的治疗效果。食管癌术后发生的7例乳糜胸于确诊当日用100mL生理盐水溶解的40mg顺铂针经胸管行胸腔内注射。结果使用顺铂胸腔内注射的7例乳糜胸患者,无论其引流量多少均于短期保守治愈。1例食管良性病变术后发生的乳糜胸未便试用,行小切口开胸治愈。结论胸腔内注射顺铂可有效治疗食管癌术后乳糜胸,方法简单实用、无痛苦。其作用机理和能否在其他原因引起的乳糜胸中试用值得进一步探讨。Objective To estimate the value of intrapleural cisplatin therapy for the chylothorax complication after esophagectomy of esophageal cancer.Methods nalyzed the 8 cases of chylothorax after thoracotomy for esophageal lesions which operated from July,2004 to June,2011 in our department.Among them 7 cases had an intrapleural cisplatin therapy（40 mg cisplatin） while identified.Results All 7 cases of chylothorax caused by esophagectomy of esophageal cancer,whether the thoracic drain more or less,be cured by this method in a short period.Another case of chylothorax,the resection of an esophageal benign tumor in a youth which didn＇t practise the therapy due to the side-effects of cisplatin,be cured by thoracic duct ligation through a mini-incision approaches.Conclusion The intrapleural cisplatin had an effect on chylothorax after esophagectomy for esophageal cancer.The features of this method were simple,practical,no more suffering.It should further probe the mechanisms of cisplatin to determine whether or not it could treat chylothorax that caused by other reasons.
