单吻合器直肠拖出式超低位直肠癌保肛术的临床应用

摘　　要：目的探讨以直肠拖出和单吻合器技术实施超低位直肠癌保肛手术的疗效。方法 2006年8月—2011年5月利用直肠拖出和单吻合器保肛技术,对32例超低位直肠癌患者行癌肿切除术。肿瘤下缘距肛缘3～6cm,平均5.2cm。Dukes分期:A期18例,B期12例,C期2例。结果 32例均成功地保留了肛门和大便控制功能。术后发生吻合口瘘3例(9.4%)。全组平均随访18个月,其中有2例(6.2%)局部复发。均未发生排尿功能障碍。76.9%的男性患者和78.9%女性患者术后性功能良好。结论直肠拖出单吻合器保肛术是安全可行的,为超低位直肠癌患者提供了新的保肛术式选择。Objective To study the effect of pull- through and single stapling technique for sphincter preservation operation in super - low rectal carcinoma. Methods The study was performed on 32 patients with super- low rectal carcinoma from August 2006 to may 2011. The distance between the anal verge and the lower margin of the tumor was 3~ 6cm（averaged 5. 2cm）,inctuding 18 patients in Dukes A stage, 12 pa- tients Dukes B and 2 patients Dukes C. Results Fecal continences were preserved successfully in all pa- tients. Only 3 cases had anastomotic leakage（9.4%）. At a median follow- up of 18 months,2 patients had local recurrences （6. 2% ）. All patients were survived. There was no urinary dysfunction in this series. The successful rates of sexual function after surgery were respectively as follows： man 76. 9% and woman 78.9 %. Conclusions Pull - through and single stapling technique for sphincter preservation operation is feasible and safe which provides an original operative style for super- low rectal carcinoma.
