摘  要:为探讨冠心病患者血浆纤溶活性变化及其与冠脉病变程度的关系,本文比较了冠心病组和正常对照组在次极量运动前后血浆组织型纤溶酶原激活物(tPA)、纤溶酶原激活物抑制剂-1(PAI-1)活性的变化,及其与心肌缺血面积的相关性。结果显示:(1)运动前冠心病组tPA及PAI-1活性水平高于正常对照组(P<0.05),运动后冠心病组PAI-1活性明显升高(P<0.05),而tPA活性水平明显下降(P<0.05);(2)心肌缺血面积与PAI-1活性呈正相关。提示冠心病患者血浆纤溶活性下降,且与心肌缺血面积相关;次极量运动可加重冠心病患者纤溶系统的损害,使其易于形成血栓,诱发或加重心肌缺血。The study is to assess the changes of fibrinolytic activity and it's relevance to the severity of coronary arterylesion and extent of myocardial ischmia area. Tissue plasminogen activator (tPA) and plasminogen activator inhibitor1 (PAI-1) activities were measured in 37 patients with coronary artery disease (Group CAD) and 27 healthy persons(Group C). The results showed (1 ) the tPA and PAI-1 activities were higher in Group CAD than in Group C beforeexercise (P<0.05). After exercise, PAI-1 activity increased (P<0.05) and tPA activity decreased (P<0.05) in GroupCAD; (2) The PAI-1 activity before exercise displayed a positive correlation with the extent ofmyocardial ischemicarea (r=0.452, P<0.01). The above results suggested that the fibrinolytic activity decreased in patients with CAD,which makes the disease vessels more susceptible to thrombus formation and to develop CAD event.
