摘  要:目的利用基于体素的形态学测量方法(VBM)及静息态功能磁共振方法(resting state-functionalMRI,rs-fMRI),研究慢性期缺血性卒中者初级运动皮层(M1)的结构损伤与功能连接的相关性。方法采用3.0TMR扫描仪对28例运动功能恢复良好的慢性期缺血性卒中患者和25例年龄、性别相匹配的正常志愿者行全脑高分辨率解剖像及静息态功能磁共振扫描。原始数据用Matlab 7.8、统计参数图(SPM)8、REST软件进行处理。结果与对照组相比,缺血性卒中组患侧M1区、丘脑的灰质体积减少。患侧M1区与其自身功能连接增强,但两者不完全重叠,连接增强区主要位于结构损伤区的背内侧。患侧M1区与双侧前岛叶、眶额回、额下回、前扣带回、左侧缘上回的功能连接减弱。结论 VBM及静息态功能连接联合的方法从形态学及功能学的角度更清楚地阐释恢复期缺血性卒中者的神经调节机制。Objective To investigate the correlation between impaired structure and brain functional connectivity within primary motor cortex by voxel-based morphometry(VBM) and resting-state functional MRI(rs-fMRI) in chronic stroke patients.Methods Twenty-eight patients and twenty-five age and gender matched healthy volunteers underwent GE 3.0T HDX MR Scanner to obtain high resolutions anatomy images in whole brain and rs-fMRI data.The difference of brain volume and resting-state functional connectivity was analyzed with Matlab7.8,statistical parametric mapping(SPM) 8 and Rest software.Results Compared to the healthy controls,the grey matter exhibiting significantly decreased volume in patients included ipsilensional primary motor cortex(M1) and ipsilensional thalamus.The functional connectivity of ipsilensional M1 increased with itself.However,they were not completely overlapped.The area in which functional connectivity increased located in dorsomedial area of the impaired structure.Functional connectivity in bilateral insular lobes,frontal operculum and temporal lobes,left supramarginal gyrus were decreased in patients.Conclusion The combination of VBM and rs-fMRI analysis should be a more efficiency approach for exploring mechanisms of motor rehabilitation in chronic stroke patients from morphology and function.
