摘 要:目的:比较胸腔积液(胸液)细胞学、胸膜活检、胸液块状凝集物(凝集块)3种病理标本的获取方法及对恶性胸液的诊断阳性率;探讨3种方法联合应用的诊断价值。方法:对44例已确诊的恶性肿瘤合并胸液做的病人,经皮针刺胸膜活检后,置入细导管引流,收集胸液中凝集块行病理学检查,同时送胸液做细胞学检查。结果:胸液凝集块阳性率为90.24%,癌组织类型确定率为100%,与胸液细胞学以及胸膜活检相比,均有显著性差异(P<0.005、P<0.01,P<0.005、P<0.05)。3种方法相加阳性率为95.12%。结论:收集胸液凝集块行病理学检查是一种诊断阳性率、癌组织类型确定率高的方法;3种方法联合应用能显著提高恶性胸液的阳性诊断率。Objective:To compare pleural effusion cytology,pleural biopsy, the coagulum of pleural effusion with their positive diag-nostic rates of malignant pleural effusion; to explore the value of the three methods combination. Methods:44 patients with documented malignant carcinoma complicated with pleural effusion underwent pleural biopsy,and had thin chest tube intu-bated to drain and the coagulum collected which was cut and checked by microscopy. At the same time,cytology was under-went . Results: The positive rate of coagulum was 90.24% , and the type diagnostic rate of cancer tissue was 100% . There were significant differences between coagulum and cytology,pleural biopsy(P<0.005,P<0.01,P<0.005,P<0.05 re-spectively). The positive rate of three methods combination was 95.12%. Conclusions:The coagulum of pleural effusion checked by microscopy had a highly positive rate for type diagnosis. The three methods combination can significantly im-prove the positive rate of malignant pleural effusion.
