光动力疗法联合超脉冲CO_2激光治疗口唇部化脓性肉芽肿的疗效

摘　　要：目的探讨超脉冲CO2激光联合5-氨基酮戊酸光动力疗法(photodynamic therapy,PDT)治疗口唇部化脓性肉芽肿的临床疗效和安全性。方法口唇部化脓性肉芽肿患者16例,局部麻醉后采用超脉冲CO2激光气化外生肉芽肿组织,5-氨基酮戊酸配制成20%溶液外涂皮损,避光封包,3 h后予激光照射30 min。每次治疗前采集图像以评价治疗效果,3个月后采用3级分类法评价治疗效果,并进行患者满意度调查。结果超脉冲CO2激光联合PDT治疗口唇部化脓性肉芽肿效果明显,15例患者经1次治疗后治愈;1例治疗有效,3个月复查时,因小面积复发予以二次治疗后,再未复发。在整个治疗过程中患者满意率为94.75%。结论超脉冲CO2激光联合PDT治疗口唇部化脓性肉芽肿疗效显著,美容效果好,不良反应小,具有很好的临床应用前景。Objective To investigate the efficacy and safety to treat pyogenic granuloma jointly with CO2 laser and 5-aminolevulinic acid photodynamic therapy（ALA-PDT）. Methods Altogether 16 patients with pyogenic granuloma were given PDT after the CO2 laser treatment.Firstly,the pyogenic granuloma was vaporized with CO2 laser.The lesions then were exposed to semiconductor laser（λ=630nm） for 3 hours after being smeared with 20% 5-aminolevulinic acid.The lesions were photographed before each treatment,and the therapeutic effects were evaluated respectively with these photos 3 months later.The therapeutic effects were rated respectively on a scale from 1 to 3,and the patient satisfaction was investigated. Results Except that one patient needed the second treatment,all the patients had a complete remission after one treatment.The 3 months＇ follow-up showed that all the 16 patients were cured. Conclusions The treatment combining PDT and CO2 laser is safe and highly effective for lip pyogenic granuloma,producing good cosmetic results in dermatologic clinic.
