精制去纤酶治疗脑梗死的疗效及相关因素分析

摘　　要：目的　观察精制去纤酶治疗脑梗死的疗效 ,并探讨影响疗效的有关因素。　方法　 50例病程在 3～72 h内脑梗死患者静脉滴注精制去纤酶 ,首剂 2 .0 u,第 3、 5、 7天各用 1 .0 u;根据临床及实验室资料对其疗效进行多因素分析。　结果　总有效率 78%,在 2 4 h内治疗的有效率为 81 .9%,与 2 5～ 72 h内治疗的疗效相比有显著性差异 ( P <0 .0 5) ,治疗后血纤维蛋白原、血浆粘度明显降低 ,多因素分析表明 :疗效除与治疗时间有关外 ,还与病情程度、血糖水平有关 ,与年龄、性别、入院时血压、血脂水平等因素无关。　结论　精制去纤酶治疗适用于病情轻～中度 ,尤其是伴高纤维蛋白原血症的脑梗死患者 ,应用越早 ,效果越明显。Objectives To evaluate the effect of Defibrase in treatment of cerebral infarction(CI) and study the influence factors  Methods 50 patients were received Defibrase intravously within 3 to 72 hours after CI, first dose was 2u,following the 3rd、5th、7th day was 1 u,According to the clinical and laboratory informations,the influence factors of effect were analysed  Results The total rate of effect was 78%,the rate of effect was 81 9% within 24 hours, which was better than within 25 to 72 hours after CI(P<0 05)  The pasma fibrinogen and the plasma viscosity decreased significant compared to before treatment Mulpitle factors analysis suggested that the effect wasen't associated with age、sex、blood pressure and blood lipid,but with the time window of treatment,degree of neurological deficit and blood sugar  Conclusions Defibrase may be effectually applied to those patients with mild and moderate CI and complicated hyperfibrinogen in the early stages
