慢性非特异性结肠炎患者伴发抑郁及相关因素分析

摘　　要：目的探讨慢性非特异性结肠炎(uc)患者伴发抑郁性情绪障碍及其相关因素,为有效干预提供依据。方法选50例慢性持续型uc患者和50名健康对照者,分别完成艾森可个性问卷(EPQ)、生活事件量表(LES)、抑郁自评量表(SDS)测查和性别、文化水平、治疗情况方面的资料收集,采用均数检验、卡方检验、相关分析、多元逐步回归分析。结果患者组抑郁性情绪障碍发生率48%,与对照组比较(x^2=19.841,P<0.01);负性生活事件发生频率方面,差异显著(x^2=4.1095,P<0.05),平均每人发生项次方面,差异有高度显著性(u=7.589,P<0.01);患者组,依SDS分为因变量进行逐步回归分析,在α=0.15水平,有N分、E分、疗效、年龄因素入选,偏回归系数(7.4688、4.6406、4.5706、4.3959),P值(0.0007、0.0341、0.0336、0.0383)。结论患者伴发抑郁性情绪障碍者达48%,其危险因素中,个性缺陷、年龄因素、治疗效果相关性紧密。Objective To survey the depressive mood disorders and related factors of chronic non-specific （ulcerative） colitis in patients to provide the basis for effective interventions. Methods 50 cases of chronic non-specific colitis patients and S0 healthy controls were selected respectively to complete EPQ, LES, SDS scale test. The data of their sex, cultural level and therapeutic aspects were also collected and the data were analyzed by means of average inspection, chi-square test, correlation analysis and multivariate stepwise regression. Results 48% of the patients showed depressive mood disorder （χ2=19. 841, P 〈 0.01）, more negative life event frequency （χ2 =4. 1095, P 〈 0.05）, average frequency for each patient （ u =7. 589, P 〈 0.01）. According to the SDS as the dependent variable in regression analysis, α=0.15, N, E, curative effect and age were selected as the related factors, with the partial regression coefficients being （7. 4688, 4. 6406, 4. 5706, 4. 3959） respectively and P-values being （0. 0007,0. 0341,0. 0336,0. 0383）respectively. Conclusion The incidence of depressive mood disorder amounts to 48% in chronic non-specific colitis patients, and the risk factors are mainly personality defects, age clinical effect.
