3种不同眼压计眼压测量值的比较研究

摘　　要：目的通过比较分析Icare回弹式眼压计(RBT)、非接触式眼压计(NCT)和Goldmann眼压计(GAT)的眼压测量结果,评价RBT的临床应用价值。方法 31例(62眼)分别用RBT、NCT、GAT眼压计测量眼压,用超声角膜测厚仪测量中央角膜厚度(CCT),然后根据GAT测得的眼压值分为低眼压组(≤21 mmHg)和高眼压组(>21 mmHg),对比分析各种眼压计的测量结果。结果在全部受测眼中RBT、GAT和NCT测得的眼压均值分别为(16.66±4.36)mmHg、(17.0±5.63)mmHg和(16.02±4.81)mmHg,平均CCT为(526.53±36.24)μm。每2种眼压计测量值比较,RBT-GAT平均差值(-0.34±3.09)mmHg(t=0.862,P=0.392);RBT-NCT平均差值(0.65±2.98)mmHg(t=1.707,P=0.093);NCT-GAT平均差值(-0.98±4.02)mmHg(t=1.928,P=0.059)。不同眼压范围比较,RBT-GAT平均差值在低眼压组和高眼压组分别为(-0.46±2.34)mmHg和(3.07±3.83)mmHg(t=3.273,P=0.005);NCT-GAT平均差值在低眼压组和高眼压组分别为(0.17±3.14)mmHg和(3.79±5.41)mmHg(t=2.389,P=0.030)。3种眼压计测量值随CCT增厚而增大,相关系数分别为r(GAT)=0.388,P=0.002;r(RBT)=0.42,P=0.001;r(NCT)=0.416,P=0.001。结论 RBT、GAT和NCT眼压测量值之间具有良好的一致性;在测量较高的眼压时,RBT和NCT测量值偏离GAT测量值较大;3种眼压计测量值随CCT增厚而增大。【Objective】 To evaluate a new method of intraocular pressure（IOP） measurement,using the Icare rebound tonometer（RBT）,by comparing the IOP values measured by RBT,Goldmann applanation tonometry（GAT） and non-contact tonometry（NCT）.【Methods】 IOP readings were obtained in 62 eyes of 31 subjects,using NCT,RBT and GAT respectively.Central corneal thickness（CCT） was obtained on all eyes with ultrasound pachymetry.All Subjects were devided into high-IOP group（IOP 21 mmHg） and low-IOP group（IOP ≤ 21 mmHg） according to their IOP values measured by GAT.【Results】 The mean IOP of RBT,GAT and NCT were（16.66±4.36） mmHg,（17.0±5.63） mmHg and（16.02±4.81） mmHg,respectively.The average of CCT was（526.53±36.24） μm.The mean IOP diferences of RBT-GAT,RBT-NCT and NCT-GAT were（-0.34± 3.09） mmHg（t =0.862,P =0.392）,（0.65±2.98） mmHg（t =1.707,P =0.093） and（-0.98±4.02） mmHg（t =1.928,P =0.059）,respectively.The mean IOP diferences of RBT-GAT in low-IOP group and high-IOP group were（-0.46±2.34） mmHg and（3.07±3.83） mmHg（t =3.273,P =0.005）.And The mean IOP diferences of NCT-GAT in low-IOP group and high-IOP group were（0.17±3.14） mmHg and（3.79±5.41） mmHg（t =2.389,P =0.030）.IOP measured with RBT,GAT and NCT increased with increasing CCT [r（GAT）=0.388,P =0.002;r（RBT）= 0.42,P =0.001;r（NCT）=0.416,P =0.001].【Conclusions】 The IOP readings obtained by RBT,GAT and NCT were in close agreement with each other.However,the discrepancies in RBT and NCT from GAT varied as IOP increased.Increasing CCT resulted in an overestimation of the IOP measurement,with three different tonometers.
