帕利哌酮治疗精神分裂症有效性和安全性的Meta分析

摘　　要：目的评价帕利哌酮治疗精神分裂症的有效性和安全性(主要考察椎体外系不良反应)。方法通过Meta分析的方法纳入17篇以利培酮或奥氮平为对照组的相关文献,进行Meta分析。结果有效性方面,比值比=1.36[95%可信区间(1.06,1.75)],P=0.02,差异有统计学意义,说明帕利哌酮的有效性显著高于对照组;安全性方面,相对于利培酮对照组比值比=0.49[95%可信区间(0.34,0.69)],P<0.0001,相对于奥氮平对照组比值比=1.64[95%可信区间(0.95,2.84)],P=0.08,差异有统计学意义,说明帕利哌酮的椎体外系不良反应情况优于利培酮而次于奥氮平。结论帕利哌酮在治疗精神分裂症方面的有效性和安全性都有一定的优势。Objective To evaluate the efficacy and safly of Paliperidone in the treatment of schizophrenia [mainly on the extrapyramidal symptoms （EPS） adverse reactions~. Methods Conducted meta analysis of 17 literatures included Risperidone or Olanzapine for the control group by the RevMan4.2 software. Results In terms of efficacy, OR=1.36 [95%CI （1.06,1.75）], P=0.02, the difference had statistically significance, which showed the efficacy of Paliperidone was significantly better than the control group. In terms of safety： Compared with the control group of Paliperidone, 0R=0.49 [95%CI（0.34,0.69）], P〈0.000 1; compared with the control group of Olanzapine, OR=1.64 [95%CI （0.95,2.84）], P=0.08. The two results had statistical significance, which illustrated the situation of the EPS adverse reactions of Olanzapine was the best one, following with Paliperidone which ranked above the Risperidone＇s. Conclusion By and large, the efficacy and safey of Paliperidone in the treatment of schizophrenia has certain advantages.
