滋胃饮配合穴位注射治疗慢性萎缩性胃炎伴肠上皮化生56例  

Effect of Decoction for Nourishing Stomach Combined with Acupoint Injection on Chronic Atrophic Gastritis Accompanied with Intestinal Metaplasia,A Report of 56 Cases

摘　　要：目的:观察滋胃饮配合穴位注射治疗胃阴不足型慢性萎缩性胃炎(CAG)伴肠上皮化生的疗效。方法:将胃阴不足型慢性萎缩性胃炎伴肠上皮化生患者92例随机分为两组。治疗组56例,内服滋胃饮配合穴位注射治疗;对照组36例,口服西药治疗。4周为1个疗程,连用2个疗程。结果:治疗组治愈30例,显效15例,有效6例,无效5例,总有效率为91.07%(95%CI=79.81%～96.96%);对照组治愈10例,显效5例,有效10例,无效11例,总有效率为69.44%(95%CI=54.39%～84.49%);两组综合疗效比较(u=3.066 4,P=0.003 8),差异有显著性意义;治疗后治疗组胃黏膜肠上皮化生改善情况优于对照组(u=1.983 2,P=0.047 5)。结论:滋胃饮配合穴位注射治疗胃阴不足型慢性萎缩性胃炎(CAG)伴肠上皮化生疗效优于口服西药,其收益为OR=0.22(95%CI=0.07～0.71),NNT=5(95%CI=2.67～17.36),且可改善胃黏膜肠上皮化生程度。To observe the curative effect of Decoction for Nourishing Stomach combined with acupoint injection on chronic atrophic gastric belonging to syndrome of stomach -yin deficiency. Methods： 92 cases with chronic atrophic gastric accompanied with intestinal metaplasia belonging to syndrome of stomach -yin deficiency were divided into two groups randomly. 56 cases in treatment group were treated with Decoction for Nourishing Stomach combined with acupoint injection, 36 ones in control group were treated with western medicine. One treatment course included 60 days. Results： In treatment group 30 eases were recovered, 15 ones excellent, 6 ones effective, and 5 ones ineffective. The total effective rate was 91.07 % （95 % CI = 79.81% - 96.96% ）. In control group 10 cases were recovered, 5 ones excellent, 10 ones effective, and 11 ones ineffective. The total effective rate was 69.44% （95% CI =54.39% -84.49% ）. There was significant difference between the two groups on curative effect （μ = 3. 0664, P = 0. 0038）. After treatment the improvement of intestinal metaplasia in stomach mucosa was better in treatment group than that in control group （μ = 1. 9832, P = 0. 0475 ）. Conclusion ： Decoction for Nourishing Stomach combined with acupoint injection has better effect on chronic atrophic gastric accompanied with intestinal metaplasia belonging to syndrome of stomach -yin deficiency than western medicine, OR =0.22 （95CI =0.07 -0.71 ）, NNT =5 （95CI =2.67 - 17.36）, which can improve intestinal metaplasia.
