49例外周T细胞淋巴瘤-非特指型临床特征与生存分析

摘　　要：目的分析外周T细胞淋巴瘤-非特指型(peripheral T-cell lymphomas-unspecified,PTCL-U)的临床特征和治疗方法,探讨PTCL-U的预后影响因素。方法对收治的49例PTCL-U患者的临床资料,回顾性分析其临床特征、治疗方法和疗效与预后的关系。结果 49例患者中17例(34.7%)获得完全反应(CR),20例(40.8%)获得部分反应(PR),总反应率为75.5%。单因素分析显示B症状、结外侵犯、临床分期、外周T细胞淋巴瘤预后指数(prognostic index for PTCL-U,PIT)与PTCL-U患者的疗效有关,PIT和是否获得CR与PTCL-U患者预后相关(P<0.05)。多因素分析显示唯一影响OS的指标则为PIT(P<0.05)。结论 PTCL-U患者常规化疗近期疗效较好,但远期生存率低。PIT是PTCL-U独立的预后因素。Objective To analyze the clinical characteristics, therapeutic approach of peripheral T-cell lymphomas- unspecified （PTGL-U）, and explore its influence factors on the prognosis. Methods Clinical characteristics , treatments and situation of prognosis of 49 patients with PTCL-U treated from 2007 to 2010 in Sichuan Cancer Hospital were en- rolled and analyzed retrospectively. Results The complete response（CR） was observed in 17 （34.7%） out of 49 pa- tients, the partial remission（PR） in 20 （40.8%） , and the overall response rate was 75.5%. In univariate analysis, 13 symptom,extranodal involvement,clinical stage and the prognostic index for PTCL-U（PIT）were important influence fac- tors of curative effect, PIT and CR achievement were correlated with the prognosis of PTCL-U（P〈0.05）. Cox multivariable analysis of potential prognostic factors predictive for achieving OS was PIT（P〈0. 05）. Conclusion Although conventional chemotherapy yielded a high response rate for PTCL-U, the long-term survival was still low. PIT was an independent prognostic factor in our study.
