摘　　要：目的建立小康型乡镇卫生院信息化建设模式并进行应用验证。方法针对信息化建设模式的拟定、运行、应用等,采用文献分析、现场调研、定性访谈、焦点小组讨论和专家咨询的方法收集有关情报和咨询意见,筛选出适宜模式进行应用验证。结果建立了县级区域信息平台、医院管理系统、疾病分析与服务系统和远程医学信息系统推广模式;应用后信息化平台建设日益完善,应用范围扩展,优化了服务流程;慢病患者随访率提高,患者依从性增强,疾病控制率提升;培训医务人员268080人次,推广应用适宜技术47项,积累远程医疗咨询案例13825例,取得显著社会效益。结论政府主导是信息化建设可持续发展的重要保证,优化医院流程是实施医院信息化建设的基础;"三中心"协调合作是提高系统运行效率的必要条件;效益评估是小康型乡镇卫生院信息化建设的推手;通过信息网络开展针对性的健康教育,能有效提高患者对医院的认同感和依从性。Objective To establish and validate a model of informatization construction in well-off township hospitals. Methods An optimal model for information construction based upon informatizatibn construction mode, operation and application was estab- lished by using methods including literature analysis, qualitative interview, group discussion and expert consultation. In order to demon- strate its validation, the model was preliminarily applied in a township hospital. Results A regional information platform at the county level, a hospital management system, a disease analysis and service system, and a remote medical information extension model were established. After application of these models, the information platform has been improved, application extended and service processes op- timized. As the results ,follow-up rate of patients with chronic diseases was increased, patient compliance enhanced and disease control rate increased. By using the models, we had also trained 268,080 medical workers, popularized and applied 47 appropriate technologies, and accumulated 13,825 remote medical consultation cases. Thus, significant social benefits have been achieved. Conclusion Government leading is an important guarantee for sustainable development of the informatization construction. Hospital process optimization is the basis for implementation of the construction. The ＇ three centers＇ coordination and cooperation are necessary conditions for improvement of system efficiency. Performance evaluation is a pusher for the construction in well-off township health center. Health education through the information network can effectively improve patients on hospital＇s sense of identity and compliance
