摘 要:目的:研究健步走对中老年肥胖人群血脂、脂肪细胞因子的影响及与脂联素基因多态性的关联,确定对运动敏感的分子标记。方法:将92名中老年(50~65岁)单纯型肥胖患者,随机分为实验组(70人)和对照组(22人),实验组根据BMI进行分组,BMI≥25为肥胖组,23≤BMI<25为超重组。通过递增负荷运动试验测定其功能能力F.C.,从而推算靶心率。有氧健步走作为主要方式,运动强度范围为开始的40%~50%F.C.至50%~65%F.C.,有效运动时间分别为40 min/次,运动频率为5次/周,运动周期为3个月。结果:经过3个月训练后肥胖组的变化与总体组相似:总胆固醇、低密度脂蛋白胆固醇显著下降(P<0.01),高密度脂蛋白胆固醇显著升高(P<0.05),脂联素水平显著升高、游离脂肪酸出现显著下降(P<0.01)。超重组的值均未出现统计学差异。TT基因型胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇指标都出现了显著性差异(P<0.05,P<0.05,P<0.01)。TG基因型低密度脂蛋白胆固醇出现显著性差异(P<0.05)。结论:健步走是有效改善中老年肥胖人群血脂含量,提高脂联素水平的有效健身方法;APM-1 T45G基因TT基因型与训练后低密度脂蛋白胆固醇指标独立关联,TT基因型可考虑作为运动降血脂的基因敏感标记。Objective: The paper is to find out how gene polymorphism sign follows the genovariation and which genetype is more sensible to exercise, by exploring the effects of walking exercise on lipids and adipocytokines, and its connection with adiponectin polymorphis. Methods: By survey and health examination, 92 people with simple obesity( aged between 50 -65)are chosen and randomly divided to EG(70) and CG(22). According to BMI, those with BMI i〉25 being adipositas, those with 23 ~〈 BMI 〈 25 being overweight. By taking the maximal cycle ergometer exercise, their functional capacity ( F. C. ) and target heart rate are to be determined. It is set that the exercise intensity is between 40% 50%F. C. and -50% -65%F. C. , the efficient time of F. C. is 40 min/time, exercise frequency is 5 times/week, the exercise circle is 3 months, all of which is to review the connection between phe-notype and gene polymorphism sign. Results : Three months after training, changes in the obese group and the overall group : TC, LDL - c significantly decreased, HDL - c significantly increased, adi- ponectin levels significantly decreased and FFA significantly increased. Overweight values were not statistically different. TI' genotype TC, LDL - c, HDL - c significant difference. TG genotype LDL - c significant difference. Conclusions:Walking exercise is effective in improving the lipid content of the middle - aged obese people, the adiponectin levels of fitness. The correlation research between aerobic walking interfering the fat of the old and the APM - 1 gene T45G polymorphism shows, TT gene type have an independent correlation with LDL - c. , thus TT gene type are capable of being the sensitive gene signal of exercise.
