摘  要:目的探讨卵巢纤维瘤的超声声像图特征及声学基础、临床特点,以提高超声诊断准确性。方法回顾性分析经手术病理证实的32例卵巢纤维瘤患者的术前超声声像图特征及临床资料。结果肿块超声检出率及定位正确率为93.7%(30/32),定性总符合率为87.5%(28/32)。28例中单侧26例,双侧2例,共查出30枚肿瘤。声像图特征:均匀低回声为主(14枚,占46.6%),低回声伴后方回声衰减(9枚,占30.0%),强弱不均匀回声(5枚,占16.7%),囊实混合回声伴钙化(2枚,占6.7%)。肿块内无血流信号或血流稀少,皆为高阻力型频谱。有1例伴胸腹水,即Meig's综合征,13例伴腹水。结论卵巢纤维瘤的超声表现多较典型,术前可做出诊断,但少数卵巢纤维瘤与其它肿瘤声像图之间也可互相交叉,需综合分析鉴别,提高诊断符合率。Objective To explore the ultrasonographic features, acoustic basis and clinical characteristics of ovarian fibroma in order to improve the ultrasonic diagnosis accuracy. Methods A retrospective analysis was conducted for the preoperative ultrasonographic features and clinical data of 32 cases of patients with ovarian fibroma confirmed by surgical pathology. Results Tumor ultrasonic detection rate and positioning accuracy was 93.7% (30/32), the total qualitative coincidence rate was 87.5% (28/32). Among 28 cases, 26 cases were unilateral, 2 cases were bilateral, with a total of 30 pieces of tumors. Ultrasonographic features: maily homogeneous low echo (14 pieces, 46.6%), low echo with rear echo attenuation (9 pieces, 30.0%), inhomogeneous echo (5 pieces, 16.7%), solid-cystic echo with calcification (2 pieces, 6. 7%). No blood flow signals or scarce blood flow were seen within the tumors, all were high resistance type spectrum. One case was with hydrothorax and ascite, namely Meig's syndrome, 13 cases with ascite. Conclusion The ultrasonic appearance of most ovarian fibroma is more typical, preoperative ultrasonic examination can make the diagnosis; the ultrasonographic appearance of a few ovarian flbroma and other tumors can also cross each other, so comprehensive analysis and identification is necessary to improve the diagnosis coincidence rate.
