摘　　要：目的探讨卵胞浆内单精子注射(ICSI)治疗周期不同的授精时间对妊娠率、种植率及流产率的影响。方法将本中心2010年行ICSI治疗的342个周期分为三组,人绒毛膜促性腺激素(HCG)注射后38～40 h行ICSI治疗的128个周期为A组,HCG后40～42 h行ICSI治疗的154个周期为B组,HCG后42～44 h行ICSI治疗的60个周期为C组,观察和比较三组的妊娠率、种植率以及流产率。结果妊娠率、种植率、流产率在A组分别为42.2%、26.8%、9.3%;B组为41.6%、26.6%、9.4%;C组为25.0%、16.1%、20.0%。A组与B组的妊娠率、种植率、流产率均无统计学差异(P>0.05),C组的妊娠率、种植率均显著低于A组和B组(P<0.05),C组的流产率高于A组和B组,但无统计学差异(P>0.05)。结论 ICSI治疗周期不同的授精时间对妊娠率、种植率和流产率均有影响,HCG注射42～44 h授精的周期,妊娠率和种植率均显著低于HCG注射38～42 h授精的周期,且流产率有增加的趋势。Objective： To investigate the impact of insemination timing for intracytoplasmic sperm injection（ICSI） on pregnancy rate, implantation rate and miscarriage rate. Methods： A total of 342 ICSI cycles, which were accomplished in our assisted reproductive technology （ART）center in 2010 ,were allocated to three groups to compare the pregnancy rate,implantation rate and miscarriage rate. ICSI was performed during 38-40 hours after HCG injection in 128 cycles in group A,40- 42 hours in 154 cycles in group B,and 42-44 hours in 60 cycles in group C. Results： The pregnancy rates, implantation rates and miscarriage rates were 42.2% vs. 41.6% vs. 25.0%,26.8% vs. 26.6% vs. 16.1%, and 9.3% vs. 9.4% vs. 20.0% in group A,group B and group C respectively. There were no significant differences between group A and group B （P ~ 0.05）. The pregnancy rates and implantation rates were significantly lower in group C compared with group A and B（P d0.05）. The miscarriage rate was higher in group C than that in group A and B, but there was no significant difference（P^0.05）. Conclusions： Insemination timing influences the pregnancy rate, implantation rate and miscarriage rate in ICSI cycles. The pregnancy rate and implantation rate are significantly reduced when ICSI is performed during 42-44 hours after HCG injection comparing with 38-42 hours, while the miscarriage rate shows a increased tendency.
