多层螺旋CT在多房性囊性肾癌中的应用价值

摘　　要：目的探讨多房性囊性肾癌的CT表现特征,提高对其诊断的准确性。方法回顾性分析经手术病理证实的多房性囊性肾癌8例。全部病例均行CT平扫及三期增强扫描。结果 8例均为多囊性,房间隔及囊壁厚度不均,2例可见壁结节。CT平扫表现为圆形或分叶状不均匀低密度影。增强扫描动脉期病变明显不均匀强化,实质期强化较动脉期稍减低,但仍较明显,肾盂排泄期强化程度减弱,而囊液在三期增强扫描中始终未见强化。结论多层螺旋CT在多房性囊性肾癌的诊断中具有一定特征性表现,在临床上具有较高的应用价值。Objective To explore CT performance characteristics of multilocular cystic renal cell carcinoma and to improve the accuracy of the diagnosis. Methods The CT findings of 8 cases of multilocular cystic renal carcinoma pathologically proved were analyzed. All cases were performed with plain CT and three phase enhancement scanning. Results All 8 cases were polycystic with the uneven thickness of interatrial septum and capsule wall, in which 2 cases presented with wall nodules. All of lesions were round or lobulated with uneven low density on plain CT scans. The lesions appeared as obvious uneven strengthening on enhancement scanning arterial phase, substantial phase slightly reduced, but still were apparent, strengthening degree was significantly reduced on renal pelvis excretion phase, and cystic fluids in three phase scans were never enhanced. Conclusion The multilocular cystic renal cell carcinoma has some characteristic CT features. MSCT has a high application value in clinic.
