摘  要:目的:探讨超声刀联合止血纱布应用对乳腺癌术后减少腋窝创面引流量的效果。方法:回顾性分析我院2010年1月至2011年6月期间224例行乳腺癌改良根治术患者的手术资料,A组(联合组)72例术中采用超声刀联合止血纱布应用,B组(止血纱布组)50例术中单纯腋窝止血纱布应用,C组(超声刀组)54例术中单纯超声刀应用,D组(电刀组)48例患者为单纯应用电刀组。比较4组患者手术时间、术中出血量、术后引流量、引流管留置时间情况。结果:C组与D组比较,C组手术时间更长但术中出血量更少,差异不显著(P>0.05),引流管留置时间及术后引流量明显减少,差异显著(P<0.05)。A组与C组比较,手术时间与术中出血量无明显差异,但引流管留置时间及术后引流量明显减少,差异显著(P<0.05)。结论:应用超声刀联合止血纱布可显著减少乳腺癌术后腋窝创面渗出,减少引流管留置时间。Objective:To study the role of ultracision harmonic scalpel and hemostatic gauze to reduce the drain- age volume of axillary wound surface after breast cancer operation. Methods: Total of 224 patients with breast cancer were included in this study between January 2010 and June 2011. Combined group (group A, n = 72) applied ultra- cision harmonic scalpel combined with hemostatie gauze, hemostatic gauze group ( group B, n = 50) used the hemo- static gauze on axillary wound surface, uhracision harmonic scalpel group (group C, n = 54 ) ,used uhracision har- monic scalpel in operation process without hemostatic gauze. Electric scalpel group ( group D) included 48 patients. Observed and recorded the operation time, blood loss during operation, drainage volume and drainage time after opera- tion. Results:In group C, the operation time was longer, but the blood loss volume was less than group D (P 〉 0.05 ), the drainage volume and drainage time was less than group D( P 〈 0.05 ) ;In group A, the operation time and blood loss volume was as same as group C, but the drainage volume and drainage time was less than group C ( P 〈 0.05 ). Conclusion: Combined application of ultraeision harmonic scalpel and hemostatic gauze could reduce the drainage volume of axillary wound surface after breast cancer operation.
