腹腔镜结直肠癌根治术后早期肠梗阻的诊治(附34例报告)

摘　　要：目的:探讨腹腔镜结直肠癌根治术后早期肠梗阻的诊断、治疗。方法:回顾性分析2001年1月至2011年10月我院结直肠肛门病外科病房行腹腔镜结直肠癌根治术术后早期出现肠梗阻的34例患者的临床资料。结果:我院2001年1月至2011年10月共行腹腔镜结直肠癌根治术患者211例,术后一月内出现肠梗阻者34例(16.11%)。其中,粘连性肠梗阻1例,麻痹性肠梗阻6例,炎性肠梗阻3例,结直肠肝转移癌切除术后肠梗阻8例,药物相关性肠梗阻6例,神经相关性肠梗阻7例,肛门闭合过紧2例,重复癌所致肠梗阻1例。本组腹腔镜结直肠癌根治术后肠梗阻发生率16.11%,其中较为常见的原因依次为结直肠肝转移癌切除术后肠梗阻、神经相关性肠梗阻、药物相关性肠梗阻与麻痹性肠梗阻。本组34例患者均经积极治疗后治愈。结论:腹腔镜结直肠癌根治术后肠梗阻是腹腔镜大肠手术后较为常见的并发症,对术后患者有针对性的预防及积极的治疗,可以避免或减轻肠梗阻对患者造成的损害,取得满意疗效。Objective：To present the clinical diagnosis, treatment of intestinal and bowel obstruction patients after laparoscopic colorectal cancer radical surgery. Methods ： The clinical data of 34 cases of early - stage intestinal and bowel obstruction after laparoseopic colorectal cancer radical surgery between January 2001 and October 2011 were analyzed retrospectively. Results： From January 2001 to October 2011,211 patients underwent aparoscopic eolorec- tal cancer radical surgery, 34 of them suffered obstruction in one month, including 1 case due to adhesive obstruc- tion, 6 cases due to paralytic obstruction, 3 cases due to inflammatory ileus, 8 cases after resection due to mataststic hepatic cancer, 2 cases due to anal tight closure, 6 cases of drug - related type, 7 cases of nerve - related type, 1 case due to multiple primary cancer. The incidence rate of early - stage intestinal and bowel obstruction after laparo- scopic colorectal cancer radical surgery was 16.11%. The common reasons were resection of mataststic hepatic canc- er, nerve -related type, drug -related type and paralytic type. Conclusion： Early -stage intestinal and colonic ob- struction was a very common complication after laparoscopic colorectal cancer radical surgery. Prevention and treat- ment in time could avoid or relieve the injuries caused by obstruction.
