慢性牙周炎患者治疗前后患牙中IL-6、IL-22及IL-4表达水平的研究

摘　　要：目的:探讨白细胞介素-6(IL-6)、白细胞介素-22(IL-22)、白细胞介素-4(IL-4)与慢性牙周炎的关系。方法:选取我院进行治疗的48例慢性牙周炎患者为治疗组,全部常规治疗,正常健康对照组32例,记录正常对照组及治疗组治疗前、后的临床指数,采用ELISA检测治疗组治疗前、后及正常对照组龈沟液(GCF)及血清中IL-6、IL-22及IL-4的含量,并讨论与临床指数的相关性。结果:IL-6、IL-22及IL-4含量与正常对照组比较,治疗组治疗前及治疗后6周差异有统计学意义(P均<0.01),治疗后12周接近正常对照组值,差异无统计学意义(P均>0.05);治疗后两组与治疗前比较,各项指标差异均有统计学意义(P均<0.01)。IL-6、IL-22及IL-4含量与牙周探诊深度(PD)治疗前、后比较均有线性显著相关(P均<0.05),与菌斑指数(PⅡ)治疗前、后比较均无线性显著相关(P均>0.05);IL-6与IL-22、IL-4与IL-22分别比较,均有线性显著负相关(P均<0.05),IL-6与IL-4含量在治疗前、后比较有线性显著正相关(P均<0.05)。结论:IL在牙周炎的免疫保护或免疫病理过程中起着重要作用。Objective- To investigate the expression level oi IL-6, IL-ZZ ana iL-4 cnromc perlouon- titis teeth before and after treatment. Method： Forty-eight patients with chronic periodontitis that were treated in our hospital were selected as treatment group, which were given conventional treatment, and 32 healthy volunteers were enrolled as control group. Concentrations of IL-6, IL-22 and IL-4 in serum and GCF as well as other clinical indexes before and after treatment were detected with ELISA Results： Signifi- cant difference in IL-6, IL-22 and IL-4 concentrations were found between the two groups, and the concen- trations of the treatment group before treatment was also significant different from that 6 weeks aftertreatment （P all d0.01）. After 12 weeks of treatment, IL-6 and IL-22, IL-4 and IL-22 levels were similar in treatment group and control group （P all 〉0.05）. Significant linear correlation was found between IL- 6 and IL-22, IL-4 and IL-22 levels and PD before and after treatment, but no similar correlation was found between those levels and P 11 （P〉0.05）. Positive linear correlation was found among IL-6 and IL-22, IL- 4 and IL-22 levels（P〈0.05）, while negative correlation was found between IL-6 and IL-4 before and after treatment （P〈0.05）. Gonclusions： IL plays an important role in protective immunity or immune patho- logical process in periodontitis.
