摘  要:目的:探讨蛛网膜下腔出血患者脑脊液中内皮素、氧合血红蛋白的表达与脑血管痉挛的关系。方法:采集74例蛛网膜下腔出血患者脑脊液标本,采集时间为入院后第1、3、7、10、14天,用放射免疫方法测定脑脊液中内皮素浓度,比色法测定脑脊液中氧合血红蛋白的浓度,TCD检测颅内主要动脉血流速度,分析内皮素及氧合血红蛋白浓度与脑血管痉挛的相关性。结果:出血后第3天脑脊液中内皮素及氧合血红蛋白浓度增高,在出血后第7天达到高峰,此后逐渐下降;发病后1~3 d大脑中动脉流速增快,第7天达峰值,第14天后明显减慢;大脑中动脉血流速度与脑脊液内皮素浓度、氧合血红蛋白浓度呈正相关(r=0.751及r=0.458)。结论:蛛网膜下腔出血患者脑脊液中内皮素与氧合血红蛋白含量与出血后脑血管痉挛程度呈正相关。Objective: To investigate the expression and concentration of endothelin and oxyhemoglo- bin in cerebrospinal fluid (CSF) of patients with subarachnoid hemorrhage, and to explore their regula tory effects on cerebral vasospasm. Methods: CSF of 74 patients with subarachnoid hemorrhage was collected on the 1st, 3rd, 7th, 10th and 14th day after subarachnoid hemorrhage. Radioimmunoassay method was applied to determine endothelin concentration in the CSF. The concentration of oxyhemo- globin in CSF was determined with colorimetry. TCD was used to measure the blood flow velocity of middle cerebral artery. The relationship of endothelin and oxyhemoglobin with cerebral vasospasm was analyzed. Results: The levels of oxyhemoglobin and endothelin in CSF remarkably increased on the 3rd day, and reached peak on the 7th day, and then decreased gradually. Middle cerebral artery blood flow velocity became faster on the lst-3rd day, reached peak on the 7th day, slowed down after the 14th day. Middle cerebral artery blood flow velocity was positively correlated with endothelin and oxy- hemoglobin concentration. Conclusions: The levels of endothelin and oxyhemoglobin in CSF of pa- tients with subarachnoid hemorrhage positively correlate with cerebral vasospasm degree.
